What is the best KVM for a programmer? I need to switch between a locked-down corporate box and my development machine rather than between servers.

I've used a Black Box four port PS/2 VGA KVM switch for many years, but with the advent of USB-only PCs and DVI I need to upgrade as it doesn't play well with USB to PS/2 converters.

My ideal features:

USB keyboard and mouse input/output
dual monitor switching
four ports, but two would do at a push
switch on middle mouse click, or from a keyboard hotkey at a pinch
