In various organizations(Software Company) there would be various designations having different roles.

I would like to know the Industry accepted & followed trend in the organization hierarchy(..Like DBA,System Architect,Project Manager,Senior Developer,Developer,QA,Design Team,Delivery Manager etc..).And the various roles played by each of them in the various stages of the Software Development Life Cycle.Who all could possibly be sharing the responsibility mutually?
