  Overview Your organization (executives, managers, and employees) understands the value of having written business process documents (process maps, procedures, instructions, reference documents, and form abstracts). Policies and procedures should be documented because they help to reduce the range of individual decisions and encourage management by exception: the manager only needs to give special attention to unusual problems, not covered by a specific policy or procedure. As more and more procedures are written to cover recurring situations, managers will begin to make decisions which will be consistent from one functional area to the next.Companies should take a project management approach when implementing an environment for a sustainable documentation program and do the following:1. Identify an Executive Champion2. Put together a winning team3. Assign ownership4. Centralize publishing5. Establish the Document Maintenance Process Up Front6. Document critical activities only7. Document actual practice8. Minimize documentation9. Support continuous improvement10. Keep it simple 1. Identify an Executive ChampionAppoint a top down driver. Select one key individual to be a mentor for the procedure planning team. The individual should be a senior manager, such as your company president, CIO, CFO, the vice-president of quality, manufacturing, or engineering. Written policies and procedures can be important supportive aids when known to express the thinking for the chief executive officer and / or the president and to have his or her full support. 2. Put Together a Winning TeamChoose a strong Project Management Leader and staff the procedure planning team with management members from cross functional groups. Make sure team members have the responsibility - and the authority - to make things happen.The winning team should consist of the Documentation Project Manager, Document Owners (one for each functional area), a Document Controller, and Document Specialists (as needed). The Tutor Implementation Guide has complete job descriptions for these roles. 3. Assign Ownership It is virtually impossible to keep process documentation simple and meaningful if employees who are far removed from the activity itself create it. It is impossible to keep documentation up-to-date when responsibility for the document is not clearly understood.Key to the Tutor methodology, therefore, is the concept of ownership. Each document has a single owner, who is responsible for ensuring that the document is necessary and that it reflects actual practice. The owner must be a person who is knowledgeable about the activity and who has the authority to build consensus among the persons who participate in the activity as well as the authority to define or change the way an activity is performed. The owner must be an advocate of the performers and negotiate, not dictate practices.In the Tutor environment, a document's owner is the only person with the authority to approve an update to that document. 4. Centralize Publishing Although it is tempting (especially in a networked environment and with document management software solutions) to decentralize the control of all documents -- with each owner updating and distributing his own -- Tutor promotes centralized publishing by assigning the Document Administrator (gate keeper) to manage the updates and distribution of the procedures library. 5. Establish a Document Maintenance Process Up Front (and stick to it) Everyone in your organization should know they are invited to suggest changes to procedures and should understand exactly what steps to take to do so. Tutor provides a set of procedures to help your company set up a healthy document control system. There are many document management products available to automate some of the document change and maintenance steps. Depending on the size of your organization, a simple document management system can reduce the effort it takes to track and distribute document changes and updates. Whether your company decides to store the written policies and procedures on a file server or in a database, the essential tasks for maintaining documents are the same, though some tasks are automated. 6. Document Critical Activities Only The best way to keep your documentation simple is to reduce the number of process documents to a bare minimum and to include in those documents only as much detail as is absolutely necessary. The first step to reducing process documentation is to document only those activities that are deemed critical. Not all activities require documentation. In fact, some critical activities cannot and should not be standardized. Others may be sufficiently documented with an instruction or a checklist and may not require a procedure. A document should only be created when it enhances the performance of the employee performing the activity. If it does not help the employee, then there is no reason to maintain the document. Activities that represent little risk (such as project status), activities that cannot be defined in terms of specific tasks (such as product research), and activities that can be performed in a variety of ways (such as advertising) often do not require documentation. Sometimes, an activity will evolve to the point where documentation is necessary. For example, an activity performed by single employee may be straightforward and uncomplicated -- that is, until the activity is performed by multiple employees. Sometimes, it is the interaction between co-workers that necessitates documentation; sometimes, it is the complexity or the diversity of the activity.7. Document Actual Practices The only reason to maintain process documentation is to enhance the performance of the employee performing the activity. And documentation can only enhance performance if it reflects reality -- that is, current best practice. Documentation that reflects an unattainable ideal or outdated practices will end up on the shelf, unused and forgotten.Documenting actual practice means (1) auditing the activity to understand how the work is really performed, (2) identifying best practices with employees who are involved in the activity, (3) building consensus so that everyone agrees on a common method, and (4) recording that consensus.8. Minimize Documentation One way to keep it simple is to document at the highest level possible. That is, include in your documents only as much detail as is absolutely necessary.When writing a document, you should ask yourself, What is the purpose of this document? That is, what problem will it solve?By focusing on this question, you can target the critical information.• What questions are the end users likely to have?• What level of detail is required?• Is any of this information extraneous to the document's purpose? Short, concise documents are user friendly and they are easier to keep up to date. 9. Support Continuous Improvement Employees who perform an activity are often in the best position to identify improvements to the process. In other words, continuous improvement is a natural byproduct of the work itself -- but only if the improvements are communicated to all employees who are involved in the process, and only if there is consensus among those employees.Traditionally, process documentation has been used to dictate performance, to limit employees' actions. In the Tutor environment, process documents are used to communicate improvements identified by employees. How does this work? The Tutor methodology requires a process document to reflect actual practice, so the owner of a document must routinely audit its content -- does the document match what the employees are doing? If it doesn't, the owner has the responsibility to evaluate the process, to build consensus among the employees, to identify "best practices," and to communicate these improvements via a document update. Continuous improvement can also be an outgrowth of corrective action -- but only if the solutions to problems are communicated effectively. The goal should be to solve a problem once and only once, which means not only identifying the solution, but ensuring that the solution becomes part of the process. The Tutor system provides the method through which improvements and solutions are documented and communicated to all affected employees in a cost-effective, timely manner; it ensures that improvements are not lost or confined to a single employee. 10. Keep it Simple Process documents don't have to be complex and unfriendly. In fact, the simpler the format and organization, the more likely the documents will be used. And the simpler the method of maintenance, the more likely the documents will be kept up-to-date. Keep it simply by:• Minimizing skills and training required• Following the established Tutor document format and layout• Avoiding technology just for technology's sake No other rule has as major an impact on the success of your internal documentation as -- keep it simple.   After installing and using Ubuntu 11.10 for while I decided to install gnome shell. After Installing and rebooting my system I saw that Ubuntu light theme (ambiance/radiance) is gone. I check /usr/shar/themes and /usr/shar/icons, it seems that the light themes and mono icons exist. I even reinstalled gtk3-engine-unico package but none of these effort fixed my problem :( my desktop now looks like this: What should I do to get my default ubuntu theme back?

• OpenGL - Calculating camera view matrix

  Problem I am calculating the model, view and projection matrices independently to be used in my shader as follows: gl_Position = projection * view * model * vec4(in_Position, 1.0); When I try to calculate my camera's view matrix the Z axis is flipped and my camera seems like it is looking backwards. My program is written in C# using the OpenTK library. Translation (Working) I've created a test scene as follows: From my understanding of the OpenGL coordinate system they are positioned correctly. The model matrix is created using: Matrix4 translation = Matrix4.CreateTranslation(modelPosition); Matrix4 model = translation; The view matrix is created using: Matrix4 translation = Matrix4.CreateTranslation(-cameraPosition); Matrix4 view = translation; Rotation (Not-Working) I now want to create the camera's rotation matrix. To do this I use the camera's right, up and forward vectors: // Hard coded example orientation: // Normally calculated from up and forward // Similar to look-at camera. Vector3 r = Vector.UnitX; Vector3 u = Vector3.UnitY; Vector3 f = -Vector3.UnitZ; Matrix4 rot = new Matrix4( r.X, r.Y, r.Z, 0, u.X, u.Y, u.Z, 0, f.X, f.Y, f.Z, 0, 0.0f, 0.0f, 0.0f, 1.0f); This results in the following matrix being created: I know that multiplying by the identity matrix would produce no rotation. This is clearly not the identity matrix and therefore will apply some rotation. I thought that because this is aligned with the OpenGL coordinate system is should produce no rotation. Is this the wrong way to calculate the rotation matrix? I then create my view matrix as: // OpenTK is row-major so the order of operations is reversed: Matrix4 view = translation * rot; Rotation almost works now but the -Z/+Z axis has been flipped, with the green cube now appearing closer to the camera. It seems like the camera is looking backwards, especially if I move it around. My goal is to store the position and orientation of all objects (including the camera) as: Vector3 position; Vector3 up; Vector3 forward; Apologies for writing such a long question and thank you in advance. I've tried following tutorials/guides from many sites but I keep ending up with something wrong.   I've been reading some literature on security, specifically password security/encryption, and there's been one thing that I've been wondering: is the 3-strike rule a perfect solution to password security? That is, if the number of password attempts is limited to some small number, after which all authentication requests will not be honored, will that not protect users from intrusion? I realize gaining access or control over something doesn't always mean going through the authentication system, but doesn't this feature make dictionary/brute-force attacks obsolete? Is there something I'm missing?

• Disk Drive not working

  The CD/DVD drive on my sisters' (I'm helping her shift from Win. XP (now officially deprecated by Microsoft) to Ubuntu) system. Now, it may end up being a failed attempt, all together (Almost the whole last year (when she's been on XP) the disk drive hasn't (not even powering on) been working.), I just want to make sure I've explored every remote possibility. Because I figure, "Huh, now that I've got Ubuntu running, instead of XP, that (just) might make a difference.". I have tried using the sudo lshw command in the terminal, to (seemingly) no avil, but, who knows, you might be able to make something out of it. • XFCE session with GNOME in background

  Hi askUbuntu I recently installed XFCE on my Ubuntu 10.04 and it runs finely but with an issue. Everytime I login into my XFCE session I see the background image of GNOME session and after few seconds it disappears which gives me the impression that XFCE session is running over an already running GNOME session. If that is really the case then it might also be eating into my system's RAM which could be a bug. All helps and suggestions are welcome.

  OK so not a customized world, but do you know what goes into creating that customized web store front for you? How do you get those additional offers from vendors when you call in for service or when you are browsing a storefront. This is what is has been happening behind the scenes.  When a customer calls in a contact center for service, at the end of the conversation, they are offered a new product, or service. But what just transpired was that the CRM system that was in place had routed the call to the right agent, the agent got the pop up screen with the customer information, and the call request  was handled. Then came the decision point to cross-sell and up-sell, The agent got some recommended offers that were created based on analyzed data (this data had been put into a data warehouse, modeled, profiled and rules were implemented e.g.. People with profile X like product Y).  But with this system, what happens is that analytics can be applied to a very small subset. Now comes Real Time Decisioning (RTD), this helps companies make optimal decisions in the context of transactional systems. It enables companies to improve business processes with real time intelligence on every single transaction. RTD is like a service plug-in that you put at the back of your transactional systems and that you  ping to get a recommendation.  It listens to business process flows and data moving through the process, getting all that data, processes all that you can do with that data, and gives out out various offers. It takes a process centric view of analytics rather than just a data centric view. It continuously observes and learns from ever-changing customer behavior and applies those insights to providing real-time decisions and recommendations at any customer touch point. At Oracle we define Real Time Decisioning as “ The solution that addresses a business issue faced by all organizations : how to make accurate decisions, using the most up to date information, in real time…consistently and in large volumes”.   It happens sometimes, but I've managed to restore the Nautilus without rebooting the system.But today, when I attached a file .svg by e-mail, he first caught the Google Chrome and then Firefox 5, and when I open the folder in Nautilus Ubuntu crashed.   Ever since I bought this laptop I've been trying to install Ubuntu on it. It came with W8 preinstalled. Up to this point, I've been able to boot in UEFI mode with a black screen. I can tell it's trying to use the nVidia card (there's a led on the computer, depending on the color you can tell which GPU is using) and if I press crtl+alt+F1 I can go to console mode. Taking this advantage I tried to install bumblebee and after a successful install the led that indicates which GPU is being used change, indicating that it switched to the Intel HD 4600 graphics. After the installation I tried to initiate the graphic interface (startx) with no success. I also saved the dsmeg output to see if it can be of any help. In order to be able to get to this stage I had to boot with nomodeset option and removed quiet and splash. Anyone got this same error? Any guidance? I've tried other linux distros and so far the only one that is able to boot is Opensuse 12.3 without any issues (but only when I switch to legacy mode instead of UEFI).
