  • How to enable Apache internal debug log (Win32)

    On Win32 I'm getting intermittent output like this at page header: order allow,deny deny from allorder allow,deny deny from all I'm running php script which has number of .htaccess files in each subfolder. I need to understand why sometimes apache is just dumping me a content of this files. So just wondering if there is any way to enable debug log. I've tried ErrorLog "e:/wamp/logs/apache_error.log" LogLevel debug However can't see any debug output neither in error nor in access logs. Thanks

  • This task is currently locked by a running workflow and cannot be edited. Limitation to both Nintex and SPD workflow

    Note, this post is from Nintex Forum here. These limitations apply to both SharePoint designer Workflow and Nintex Workflow as Nintex using the SharePoint workflow engine. The common cause that I experience is that ‘parent’ workflow is generating more than one task at once. This is common as you can have multiple approvers for certain approval process. You could also have workflow running when the task is created, one of the common scenario is you would like to set a custom column value in your approval task. For me this is huge limitation, as Nintex lover I really hope Nintex could solve this problem with Microsoft going forward. Introduction “This task is currently locked by a running workflow and cannot be edited” is a common message that is seen when an error occurs while the SharePoint workflow engine is processing a task item associated with a workflow. When a workflow processes a task normally, the following sequence of events is expected to occur: 1.       The process begins. 2.       The workflow places a ‘lock’ on the task so nothing else can change the values while the workflow is processing. 3.       The workflow processes the task. 4.       The lock is released when the task processing is finished. When the message is encountered, it usually indicates that an error occurred between step 2 and 4. As a result, the lock is never released. Therefore, the ‘task locked’ message is not an error itself, rather a symptom of another error – the ‘task locked’ message does not indicate what went wrong. In most cases, once this message is encountered, the workflow cannot be made to continue and must be terminated and started again. The following is a guide that can help troubleshoot the cause of these messages.  Some initial observations to narrow down the potential causes are: Is the error consistent or intermittent? When the error is consistent, it will happen every time the workflow is run. When it is intermittent, it may happen regularly, but not every time. Does the error occur the first time the user tries to respond to a task, or do they respond and notice the workflow does not continue, and when they respond again the error occurs? If the message is present when the user first responds to the task, the issue would have occurred when the task was created. Otherwise, it would have occurred when the user attempted to respond to the task. Causes Modifying the task list A cause of this error appearing consistently the first time a user tries to respond to a task is a modification to the default task list schema. For example, changing the ‘Assigned to’ field in a task list to be a multiple selection will cause the behaviour. Deleting the workflow task then restoring it from the Recycle bin If you start a workflow, delete the workflow task then restore it from the Recycle Bin in SharePoint, the workflow will fail with the ‘task locked’ error.  This is confirmed behaviour whether using a SharePoint Designer or a Nintex workflow.  You will need to terminate the workflow and start it again. Parallel simultaneous responses A cause of this error appearing inconsistently is multiple users responding to tasks in parallel at the same time. In this scenario, one task will complete correctly and the other will not process. When the user tries again, the ‘task locked’ message will display. Nintex included a workaround for this issue in build 11000. In build 11000 and later, one of the users will receive a message on the task form when they attempt to respond, stating that they need to try again in a few moments. Additional processing on the task A cause of this error appearing consistently and inconsistently is having an additional system running on the items in the task list. Some examples include: a workflow running on the task list, an event receiver running on the task list or another automated process querying and updating workflow tasks. Note: This Microsoft help article (http://office.microsoft.com/en-us/sharepointdesigner/HA102376561033.aspx#5) explains creating a workflow that runs on the task list to update a field on the task. Our experience shows that this causes the ‘Task Locked’ issues when the ‘parent’ workflow is generating more than one task at once. Isolated system error If the error is a rare event, or a ‘one off’ event, then an isolated system error may have occurred. For example, if there is a database connectivity issue while the workflow is processing the task response, the task will lock. In this case, the user will respond to a task but the workflow will not continue. When they respond again, the ‘task locked’ message will display. In this case, there will be an error in the SharePoint ULS Logs at the time that the user originally responded. Temporary delay while workflow processes If the workflow is taking a long time to process after a user submits a task, they may notice and try to respond to the task again. They will see the task locked error, but after a number of attempts (or after waiting some time) the task response page eventually indicates the task has been responded to. In this case, nothing actually went wrong, and the error message gives an accurate indication of what is happening – the workflow temporarily locked the task while it was processing. This scenario may occur in a very large workflow, or after the SharePoint application pool has just started. Modifying the task via a web service with an invalid url If the Nintex Workflow web service is used to respond to or delegate a task, the site context part of the url must be a valid alternative access mapping url. For example, if you access the web service via the IP address of the SharePoint server, and the IP address is not a valid AAM, the task can become locked. The workflow has become stuck without any apparent errors This behaviour can occur as a result of a bug in the SharePoint 2010 workflow engine.  If you do not have the August 2010 Cumulative Update (or later) for SharePoint, and your workflow uses delays, “Flexi-task”, State machine”, “Task Reminder” actions or variables, you could be affected. Check the SharePoint 2010 Updates site here: http://technet.microsoft.com/en-us/sharepoint/ff800847.  The October CU is recommended http://support.microsoft.com/kb/2553031.   The fix is described as “Consider the following scenario. You add a Delay activity to a workflow. Then, you set the duration for the Delay activity. You deploy the workflow in SharePoint Foundation 2010. In this scenario, the workflow is not resumed after the duration of the Delay activity”. If you find this is occurring in your environment, install the October CU, terminate all the running workflows affected and run them afresh. Investigative steps The first step to isolate the issue is to create a new task list on the site and configure the workflow to use it.  Any customizations that were made to the original task list should not be made to the new task list. If the new task list eliminates the issue, then the cause can be attributed to the original task list or a change that was made to it. To change the task list that the workflow uses: In Workflow Designer select Settings -> Startup Options Then configure the task list as required If any of the scenarios above do not help, check the SharePoint logs for any messages with a category of ‘Workflow Infrastructure’. Conclusion The information in this article has been gathered from observations and investigations by Nintex. The sources of these issues are the underlying SharePoint workflow engine.   • PHP/APC fatal error, apc_mmap: mmap failed

    I'm seeing some intermittent CPU usage spikes to 100%, sort of correlated to these log entries: [27-Feb-2012 13:29:29] PHP Fatal error: PHP Startup: apc_mmap: mmap failed: in Unknown on line 0 [27-Feb-2012 13:29:30] PHP Fatal error: PHP Startup: apc_mmap: mmap failed: in Unknown on line 0 [27-Feb-2012 13:29:31] PHP Fatal error: PHP Startup: apc_mmap: mmap failed: in Unknown on line 0 [27-Feb-2012 13:29:31] PHP Fatal error: PHP Startup: apc_mmap: mmap failed: in Unknown on line 0 phpinfo() indicates that APC is set up, and as far as I can tell this error doesn't cause visible 500 errors on the live site, which is a WordPress install that gets about 600k views monthly. Google's been unhelpful so far, and I was hoping that someone here had some insight as to what's causing this and how to fix it. Curiously, this error only shows up /usr/local/apache2/logs/error_log and not the error_log for the cpanel-configured site.

  • Broadcom NIC Teaming with Hyper-V R2

    I have read a few contradictory articles regarding support or at least configuration with teaming of Broadcom NIC's and Hyper-V. I have a pair of Dell R805's with 4 Broadcom 5708 NIC's in them. I have teamed them up (one team for public connections and one for private connections) and I have installed Hyper-V. I am getting some intermittent connection issues though and I'm concerned that it might be due to the teaming. Basically I am wondering if anyone has successfully setup NIC teaming in the parent partition and used the teamed adapters through the Hyper-V switch. If so, what is the configuration/proper steps that you used to configure this. I have the server running Windows Server 2008 R2. Right now the server is in test mode, but my concern is that we won't see any issues until we move it into production and get some heavy traffic on the ports.

    I am currently stuck with a installation error and I was hoping that you guys would have some insight. The error is intermittent and seems to occur only in 10.7.   • open_basedir vs sessions

    On a virtual hosting server I have the open_basedir set to .:/path/to/vhost/web:/tmp:/usr/share/pear for each virtual host. I have a client who's running WordPress and he's complaining about open_basedir errors thus: PHP WARNING: file_exists() [function.file-exists]: open_basedir restriction in effect. File(/var/lib/php/session/sess_42k7jn3vjenj43g3njorrnrmf2) is not within the allowed path(s): (.:/path/to/vhost/web:/tmp:/usr/share/pear) So the PHP session save_path isn't included in open_basedir but sessions across all sites on the server seems to be working fine apart from in this intermittent instance. I thought that perhaps the default session handler ignored open_basedir and this warning was caused by WP accessing the session file directly. However from what I can see PHP 5.2.4 introduced open_basedir checking to the session.save_path config: http://www.php.net/ChangeLog-5.php#5.2.4 (I am on PHP 5.2.13). Any ideas?

  • Gmail page works but won't stop loading

    When I open the GMail page in Chrome (9.0.597.102) on OS X (10.6.4), the page works fine and is functional, but it often remains in a "Loading" state, with the loading icon active, for extremely long periods of time. Functionally, there is no problem, since everything is accessible, and the problem is intermittent. Sometimes it immediately completes loading, other times it will claim to be "loading" for hours. My date is correct (as set with time.*.apple.com), and I don't have any Google Labs enabled. Does anyone else have this problem, or know the cause and solution?

  • Windows Server 2008 Task Scheduler Tasks Not Executing

    I've been having an intermittent problem for some time now with the Windows Task Scheduler that I can't work out. I use the task scheduler to run a C# app I've written that has various plugins used to ensure production systems are working. This task scheduler itself is actually a production system so I have one simple task that executes every 8 minutes to notify an external monitoring system that the task scheduler is still up. If this external service fails to receive an "all-clear" at least once every 15 minutes (or so I don't remember the exact number right now) it will message us that the monitoring system is down. In the past we've had intermittent "down" messages from time to time and each time I've investigated the cause I was unable to find any problems. So this time I wanted to ask the StackOverflow community since it doesn't look like I'll have luck on my own. This morning at 2:32 AM the task fired (exactly 8 minutes after the previous firing) however the task didn't fire again until 3:28. There are no errors that I can see in the Event Viewer at this time. When I look at the Task Scheduler log there are no errors there either. I also have two other C# apps that run and check something each hour on the hour using task scheduler. If I look at the history for those I can see that they didn't execute at 3 AM either! They all waited until 3:28 to start as well. If I look at "tasks completed" in the Event Viewer it shows that only one task was able to run between the 2:32 AM to 3:28 AM time period.   • Wireshark (WinPCap) does not see Intel X520-DA2 10 GbE NIC teaming intermittently

    I am running a team of two 10 GigE ports on Intel X520-DA2 network card. They work well in tandem and achieve the desired throughput. However, I see an intermittent issue whereby WireShark and my own application (using WinPCap) only show the underlying ports, failing to recognize the team adapter. Details: Intel 17.4 NIC drivers on Windows Server 2008 R2 with all patches. HP DL370 G6 server. RSS enabled on Intel both underlying Intel NICs. The exact error: Unable to open the adapter (rpcap://\Device\NPF_{401D5903-16E7-41DC-8484-5D96765B9692}). failed to set hardware filter to promiscuous mode

  • DNS Settings in Cisco Wireless Lan Controllers

    The WLC is a 2100 series and I have looked back and forth through the entire web interface. Originally the idea was that the gateway should be the DNS controler and the WLC pointed directly at the gateway. For some reason, random clients are having DNS specific issues when it comes to seeing certain shares around the network. The problems are also very intermittent. Aside from going to each box and implementing a static DNS is there ACTUALLY a DNS setting lodged into the WLC settings (and I am being a numpty and can't find them) or am I stuck with performing the config 1 by 1?

  • Virutal Machine loses network connectivity on Hyper V

    We're running a number of VMs on a 6 node failover cluster of blades using Hyper V. We have an intermittent issue (every few days at different times - not a fixed frequency) of VMs losing network connectivity. Console access to the VM suggests all is fine and the underlying blade has normal connectivity. To resolve the problem we either have to re-start the VM or, more usually, we do a live migration to another blade which fires up connectivity and we then migrate it back to the original blade. I've had 3 instances of this happen with a specific VM running on a particular blade however it has happened once with a different VM running on a different blade. All VMs and blades have the same basic setup and are running Windows 2008 R2. Any ideas where I should be looking to diagnose the possible causes of this problem as the event logs provide no help?

  • Codec problem: Video noise / squares blinking as it play

    Its a weird and intermittent problem, it comes and goes with time. I thought I could fix it rebooting, but it seems that is not always. Take a look: This noise keeps blinking at the screen the entire movie. If I give up and try again an hour from now, it will be fine... weird! I have K-Lite Codec Pack installed, in my machine and many others. Its very cool, everything works perfect... but mine is the only one that does it. Do anybody know whats going on? Edit: Apparently they've just release the very latest version of K-Lite today (lol), with some bug fixes. I'll be testing it. Last time I reainstalled K-Lite the problem was gone, but came back again...

  • When DNS doesn't cache

    We've had some odd DNS problems over the past couple of days that I don't fully understand. Some of our DNS names stopped resolving for some of our customers due to some 'unknown' server reconfiguration at our DNS provider. The problem seemed to be intermittent i.e. stopped working and started working within a few minutes over a couple of days. I'm no expert on DNS, but I'd have expected DNS caches to prevent this sort of thing from happening - when we need to change an IP address for a DNS record, it can take 24 hours to propogate, so how can our DNS provider be breaking name resolution intermittently for our customers so easily? Shouldn't the DNS caches kick in here? We had a similar problem about a month ago when one of their nameservers 'decided to reload the DNS database from scratch' - this broke our name resolution too. Again, why didn't the caches satisfy the name resolution requests. Any guesses would be appreciated. John

  • Causes of hard crashes on Windows XP and how to debug

    I am occasionally seeing hard lockups on XP: totally unresponsive to keyboard/mouse, screen freezes at time of crash, no SSH/VNC possible. Very intermittent, nothing in the logs. I never see a blue screen on any kind of error message. This morning I logged in via VNC, logged out again, 20 minutes later physically sat at PC and it crashed around the time of VNC logout. I tend to suspect video cards in these kind of situations but it's a modern-ish card with modern drivers (one revision back, but this has been happening for 5 revisions or more) and normally would at least see a blue screen I expect. What would you suspect? Where can I look or what can I set up for more information? Bear in mind that this happens about once every 3 or 4 weeks, so extensive logging or intrusive monitoring isn't really an option.

    I've been running Windows XP mode on my Windows 7 Laptop for a while now and I've got my VM set up just how I like it. I am switchign laptops due to an intermittent hardware failure. Is it possible to transfer my Windows XP mode VM to my new laptop? I know it's just a Windows Virtual PC VM, how would I go about transferring it if this is possible? Will the Fact that I'm moving from Windows 7 Ultimate to Windows 7 Pro make a difference? AFAIK they both support Windows XP Mode.

  • Instant messanger capable of offline messaging & tolerant of network interruptions

    I am looking for an instant messaging solution to facilitate communications between recovery vehicles in remote rural areas. All the vehicles have internet connections, but they are intermittent depending on location. Ideally we'd like something that has the following features: Offline messaging: messages sent to clients who are offline will be delivered when they next come online, regardless of whether the sender is still online or not. Lightweight: CPU cycles are limited on the machines in these vehicles. A bloated solution will be an issue. Client platform is primarily win32, but support for osx/linux/mobile devices would be a bonus. Non-chatty: Bandwidth is a precious commodity for us, so services which use a minimal amount are ideal. Fault tolerant: We see plenty of packetloss and high latency, so whatever we use needs to be able to function in trying network conditions. I'm not fussed if we use a hosted platform like gtalk/skype/msn/icq/whatever, and likewise I can run a server if need be. Suggestions would be appreciated!

  • DNS Propagation Investigation Techniques?

    So I changed the nameservers to veeter.com about 5 days ago now - and the domain is still not resolving over the net. It does intermittently, but then cuts out again every few hours. I run 10+ other domains through the same dedicated server (running CentOS/WHM) and they are all up - so it has nothing to do with that. I have asked my dedicated server support staff what the problem might be, they told me to "log into WHM and click on 'Add a DNS Zone' under the 'DNS Functions' section on the left hand side of the screen. Once you have done this and dns fully propagates it should resolve any intermittent dns issues you may be experiencing." BUT there is already a DNS zone in place for the domain on my server - so what should I do

  • Why would a server not send a SYN/ACK packet in response to a SYN packet

    Lately, we've become aware of a TCP connection issue that is mostly limited to mac and Linux users who browse our websites. From the user perspective, it presents itself as a really long connection time to our websites (11 seconds). We've managed to track down the technical signature of this problem, but can't figure out why it is happening or how to fix it. Basically, what is happening is that the client's machine is sending the SYN packet to establish the TCP connection and the web server receives it, but does not respond with the SYN/ACK packet. After the client has sent many SYN packets, the server finally responds with a SYN/ACK packet and everything is fine for the remainder of the connection. And, of course, the kicker to the problem: it is intermittent and does not happen all the time (though it does happen between 10-30% of the time) We are using Fedora 12 Linux as the OS and Nginx as the web server.

    Looking through the event logs on my web server I noticed a strange pattern. The following events have been occurring over and over again, always back to back: The start type of the IPsec Policy Agent service was changed from demand start to auto start. The start type of the IPsec Policy Agent service was changed from auto start to demand start. Each one produces event id 7040 from the Service Control Manager. And sometimes this will happen 20 times in one minute. Any idea what would cause this? I've been trying to pinpoint an intermittent performance problem for the past several days and this is the most peculiar thing I've found so far. I'm running Windows Server 2008, SQL Server 2008, and ASP.NET 3.5 w/ MVC 1.

  • Logon script in Active Directory

    I am having a weird intermittent issue for only some users. I have a logon script that maps shared drives and issues a diisclaimer everytime the user logs on to a machine. The problem lately is that the logon script will not run for the user so we get a help desk call. When one of the help desk techs log on to the machine the script then comes up. The tech then logs off and the user will log back in and they will get the logon script. I am at wits end with this issue. Any help would be appreciated. It has happend on both Windows XP and Vista 64.

  • Solaris disk I/O spike... which monitoring tools do I need?

    Hi all, My Solaris 10 websever / database server disk io keeps spiking at intermittent times. Using iostat -xtc 5 the reads / sec will jump from 3.0 to 1450.0 and %busy will jump to 98% The apache access log doesn't indicate anything out of the ordinary. In other words, requests aren't any higher than usual. top doesn't generate anything useful. The cpu usage is fine with mysql using about 20% and nothing else to speak of really. What monitoring tool should I be using to see what process is using excessive disk I/O? or if there are any other suggestions I'm all ears. Thanks

  • Dell Latitude E4300 TouchPad - Odd Behavior

    I'm seeing some intermittent behavior on the touchpad. First it wasn't working at all. Then during restart, for a little while it seemed like the buttons attached to the joystick and external mouse were both swapped left/right. Then I went in to the dell touchpad control and disabled the touchpad and re-enabled it and nothing happened. Then a short while after disabling the joystick the touch pad started working. So I restarted to see if it would keep the settings and before login, the touchpad appeared to work, then after login it stopped working for a few minutes while login continued. Then after the login was almost completed it started to work again. It appeared to have retained the settings for the disabled joystick. It seems like perhaps there are conflicting mouse drivers trying to simultaneously or similar. This doesn't seem normal for the touchpad to be disabled for so long after login. What steps should I take to ensure the drivers are right?

  • SharePoint Designer not syncing consistently

    I've got a user who uses SharePoint Designer to maintain an internal intranet site. When syncing (remote-to-local) it appears to work at first, but usually hangs about 2-3 minutes into the sync, when he's syncing it to a sub-folder of his "My Documents". In this case, his "My Documents" is stored on a network share/profile. When I do the same thing, it works for me. The difference? My "My Documents" folder is locally stored. In other words, he's syncing from the remote server, into a network share. I'm syncing from the remote server, into a local drive. Any idea why having the sync destination on a network share, vs. a local drive would cause this? When it locks up, we can navigate to his "My Documents" folder still - so I don't believe that we're loosing connection to his drive - unless perhaps the connection is intermittent, and SharePoint Designer isn't re-trying the sync.

  • Fortigate 200A firewall CPU high resource usage

    This morning I'm receiving complaints from several end users that saying their whole department network are slow and have intermittent. Therefore I've checked our firewall to see whether if something goes wrong with the device.From my observation in the FortiGate dashboard status, the CPU resources is very high (99 percent). My first assumption is to clear the log since in the alert log the Fortigate log mention that it is already 90% full.Based on my understanding,the log can be cleared by restarting the firewall. After restarting the firewall the network seems okay again but then after several minutes it went up again.The condition still persist until now. Can someone show me where else I can check to fix this issue? I've really appreciate any help that I can get here.Thanks. Edit: diag sys top command
