• Challenges and Opportunities to Drive Change in the Healthcare System Explored at America’s Health Insurance Plans Exchange Conference and Institute 2013

  The program theme at the June America’s Health Insurance Plans (AHIP) Exchange Conference and AHIP’s Institute 2013 was Transforming Our Health Care System: Navigating and Succeeding in the New Marketplace.  Topics included care delivery transformation, innovation for a new healthcare eco system, Health Insurance Exchanges, the nexus of consumerism, retail and healthcare, driving value through improved operations and leveraging technology, data and innovation to transform care. Oracle participated as a sponsor of both conferences, signaling the significant investment and activity Oracle continues to make in helping health plans, providers and government agencies become more efficient and more relevant in the healthcare market place. AHIP is a national trade association representing the health insurance industry. AHIP’s members provide health and supplemental benefits to more than 200 million Americans through employer-sponsored coverage, the individual insurance market and public programs such as Medicare and Medicaid.   AHIP advocates for public policies that expand access to affordable health care. Health plans are focusing on the Health Insurance Exchanges and the opportunities they offer to provide better access and higher quality healthcare.  With the opportunities come operational challenges to implementation and innovative technology solutions to consider.   At the Exchange Conference, Oracle hosted a breakfast symposium on “Strategies for Success:  Driving Business Transformation in the Growing Health Insurance Exchange Market”. With Health Insurance Exchanges as catalysts for change, attendees learned about how to achieve integration within an Exchange and deploy new business strategies to support health reform initiatives. Discussion covered steps and processes to successfully establish and implement enrollment systems, quote to card activities, program pricing, claims billing, automated claims processing and new customer service tools. Piyush Pushkar, COO of Benefitalign, an Oracle partner that provides solutions to adopt innovative business models for retail, HIX, consumer-centric health plan and benefits administration, spoke on the state of the Exchanges in the U.S. and the activities health plans are engaged in to support individuals entering the healthcare system, including sales automation, member enrollment automation/portals and integration strategies with the Exchanges. The Oracle and Benefitalign partnership allows seamless integration between a health plan enrollment solution with the HIX individual market and allows for the health plan to customize and characterize the offerings available to the HIX that may or may not be available through other channels.  This approach can benefit the health plan through separation of interests, but also because some state-run HIXs require such separation. Janice W. Young, Program Director, Payer IT Strategies, IDC Health Insights, reviewed a survey of health plans on their investment priorities for this last year as well as this year.  She also identified the 2013-2015 strategies of go/get to market with front end and compliance investments; leveraging existing business processes and internal technologies; and establishing best practices.  Of key interest to the audience was a reform era payer solutions platform overview mapping technologies to support the business operations. David Bonham of the Oracle Health Insurance organization moderated the panel and spoke on Oracle’s presence in healthcare and products for payers to help them drive efficiencies and gain a competitive advantage in an ever changing market. • Colour scheme for editor - guidelines or medical reccomendations

  Is anyone aware of any studies on what colour scheme is best for use in multi-coloured text based computer work? Specifically in terms of reducing eye strain. For instance is a black back ground and light text best? Should it be a dark colour rather than going all the way to black? I've seen the questions on this site about "which is your favourite" that is not what I am after. I also aware that my question may be to specific asking for a colour scheme, if anyone could link me to some guidelines instead that would be appreciated as well. I'm concious of the fact that anyone using a computer is really using it for text based work but with the multitude of colours used to convey information within our modern IDEs I feel this is a good StackExchange site for this question.

• Health problem of a programmer

  Hi all, I've been annoyed by this fingers ache for quite a long time, my fingers ache because of too much mouse clicking during office hour plus play games after work. I forget game for a while and my fingers are getting better, but still my right pointing finger would feel pressure when I click the mouse. I haven't go to a doctor because I afraid the fee would be high and he would just suggest me too get rest for the fingers, also, I don't know what kind of doctor should I go and see. My fingers get less pressure if I use my expensive deathadder ( what a shame, I bought this for gaming, but now I use it for rest ) at home because its buttons are softer, however I cannot have such expensive mouse at my office because I am afraid people would steal it. I use some trick when I am using the mouse such as single-click open a file, adding more shortcuts at desktop for common jobs, do you guys have some other tips for me? Thank you.

• Anyone using NoSQL databases for medical record storage?

  Electronic Medical records are composed of different types of data. Visit information ( date/location/insurance info) seems to lend itself to a RDMS. Other types of medical infomation, such as lab reports, x-rays, photos, and electronic signatures, are document based and would seem to be a good candidate for a 'document-oriented' database, such as MongoDB. Traditionally, binary data would be stored as a BLOB in a RDBMS. A hybrid approach using a traditional RDBMS along with a 'document-oriented' database would seem like good alternative to this. Other alternative would be something like DB2 purexml. The ultimate answer could be that 'it depends', but I really just wanted to get some general feedback/ideas on this. Is anyone using the NoSql approach for medical records?

What is the best platform to develop the software in ie. language and database?   Hi, I have a Brocade SAN Fabric Switch, a 200E. When using the web based management interface "SwitchExplorer" I can click the port, and I see "Port Administration Services". In the first screen of data that appreas, there's a row called "Health" which has value "Unmonitored". Do you know how to set this port to be Monitored? And if also - what "Health" monitoring does? • Unusual request URL in ASP.NET health monitoring event

  I’m seeing a rather strange occurrence in the request information section of an ASP.NET health monitoring email I hope someone can shed some light on. This is a publicly facing website which runs on infrastructure at an Indian hosting provider. Health monitoring is notifying us of server errors via automated email but every now and then the requested URL appears as a totally different website. For example: Request information: Request URL: http://www.baidu.com/Default.aspx Request path: /Default.aspx User host address: 221.13.128.175 User: Is authenticated: False Authentication Type: Thread account name: NT AUTHORITY\NETWORK SERVICE Obviously the site in question is not Baidu and obviously this attribute is not the referrer either; the “Request URL” value is the path which has generated the error. The IP address is located in Beijing (coincidental given the Baidu address?) and in this instance it looks like the SQL server backend was not accessible (I haven't included the entire error message for security's sake). What would cause the request URL attribute to be arbitrarily changed to that of another site? I’ve never seen this occur in a health monitoring event before. Thanks!

• RDA Health Checks for SOA

  What is a health check in RDA? A health check evaluates something in your environment to determine whether a change needs to be considered in order to avoid a problem or optimize fuctionality. Examples of what this 'something' might be are: Configuration Parameters JVM Options Runtime Statistics What have we done for SOA? In the latest release of RDA, 4.30, we have added a Rule Set for SOA called 'Oracle SOA 11g (11.1.1) Post Installation (Generic)'. This Rule Set contains 14 SOA related health checks. These checks were all derived from common issues / solutions we see in support of the SOA product. Many of the recommendations come from the product documentation while others are covered in the SOA Knowledge Base. Our goal is that you will be able to easily identify the areas of concern and understand the guidance available from the output of the Rule Set. Running the health checks for SOA The rules that the checks use are installed with RDA and bundled by product or functional area into what are called 'Rule Sets'. To view the available Rule Sets simply run the command from the RDA home location: rda.cmd (or .sh) -dT hcve This will bring up a list of the available HCVE (Health Check / Verification Engine) Rule Sets. Each Rule Set contains a group of related rules that are used for evalutation and display of results. A rule can be considered synonymous with a single health check and they are assigned an ID, Name and Description that can be seen when they are executed. The Rule Set for SOA is option number 11 and you just enter this selection at the prompt. The Rule Set will then execute to completion. After running an HCVE Rule Set the tool will write the output to the RDA_HOME/output folder. The simplest way to view the output is to drag the .htm file to a browser but of course it can also be uploaded to a Service Request for evaluation by Oracle Support. Many of the Rule Sets will prompt you for information before they can execute their rules but the SOA Rule Set will identify the SOA domains configured in your RDA setup.cfg file. This means that you don't need to answer all of the questions again about where stuff is but it also means that you must have configured RDA for SOA. To run the Rule Set: Download the latest version of RDA from MOS Doc ID 314422.1 Configure RDA for your SOA domains. Detailed steps can be found here In it's simplest form the command is 'rda.cmd (.sh) -S SOA' Go to the RDA home location and enter the command 'rda.cmd (or .sh) -dT hcve' Select option '11' It should be noted that this our first release of a SOA Rule Set so there will probably be some things we need to clean up or fix. None of these rules will actually modify anything on your system as they are read only and do the evaluations internally. Please let us know if you have any issues with the rules or ideas for new ones so we can make them as useful as possible. The Checks Here is a list of the SOA health checks by ID, Name and Description. ID Name Description A00100 SOA Domain Homes Lists the SOA domains that were indentified from the RDA setup.cfg file A00200 Coherence Protocol Conflict Checks to see if you have both Unicast and Multicast configured in the same domain. Checks both the setDomainEnv and config.xml entries (if it exists). We recommend Unicast with fully qualified host names or IP addresses. A00210 Coherence Fully Qualified Host Checks that the host names are fully qualified or that IP addresses are used. Will fail if unqualified host names are detected. A00220 Unicast Local Host Checks that the Coherence localhost is specified for use with Unicast A00300 JTA Timeout Checks that the JTA timeout is configured for the domain and lists the value. The bundled rule will only list the current values of the JTA timeout for each SOA Domain. In the future the rule with fail with a warning if the value is 300 seconds or lower. It is recommended that timeouts follow the pattern 'syncMaxWaitTime' < EJB Timeouts < JTA Timeout. The 300 second value is important because the EJB Timeouts default to 300 seconds. Additional information can be found in MOS Doc ID 880313.1. A00310 XA Max Time Checks that the JTA Maximum XA call time is set for the domain. Fails if it is not explicitly set or if the value is less than or equal to the default of 12000 ms. A00320 XA Timeout Checks that the XA timeout is enabled and that the value is '0' for the SOA Data Source (SOADataSource-jdbc.xml) A00330 JDBC Statement Timeout Checks that the Statement Timeout is set for all SOA Data Sources. Fails if the value is not set or if it is set to the default of -1. A00400 XA Driver Checks that the SOA Data Source is configured to use an XA driver. Fails if it is not. A00410 JDBC Capacity Settings Checks that the minimum and maximum capacity are equal for all SOA Data Sources. Fails if they are not and lists specifically which data sources failed. A00500 SOA Roles Checks that the default SOA roles 'SOAAdmin' and 'SOAOperator' are configured for the soa-infra application in the file sytem-jazn-data.xml. Fails if they are not. A00700 SOA-INFRA Deployment Checks that the soa-infra application is deployed to either a cluster, all members of a cluster or a stand alone server. A00710 SOA Deployments Checks that the SOA related applications are deployed to the same domain members as soa-infra. A00720 SOA Library Deployments Checks that the SOA related libraries are deployed to the same domain members as soa-infra. A00730 Data Source Deployments Checks that the SOA Data Sources are all targeted to the same domain members as soa-infra

But after the year 2014, health insurance companies will not be able to turn customers away for preexisting conditions. Why wouldn't someone pay the fine or buy low end coverage until they got ill and supplement the coverage or buy an extensive plan?

• Triggering State Changes with Health Counter

  I'm developing a game where the player changes states as their health decreases. Below 50, it should trigger animation1. Below 30, it should trigger animation2. The problem is, I only want to trigger animation1 once. But my game timer is checking every "frame", so it's triggering animation1 every cycle below 50. I only want it to trigger once, then not again until it's gone over 50 and then naturally decreased back to below 50. Are there any tried and true strategies for triggering state changes as a timer counts down (without the over-triggering problem)? I thought I could say: if (health == 50) animation1.play(); but sometimes, health never equals exactly 50, so it will skip right past that statement.
