  • iiR Hospital Digital 2011: Tras la historia digital ¿qué?

    Como el acceso a la documentación está restringido, sólo voy a comentar por encima algunos temas o planteamientos que me han llamado la atención del VI Foro Hospital Digital 2011, organizado por iiR. Y comienzo destacando la buena moderación de Maribel Grau del Hospital Clínic de Barcelona que estuvo sobria, eficaz y motivadora del debate. Me impresionó el proyecto Hospital Líquido del Hospital San Joan de Deu de Barcelona por el compromiso corporativo con una medicina colaborativa involucrando a los pacientes y a los profesionales, con unas iniciativas de eSalud y Salud2.0 avanzadas y apoyadas en un buen soporte legal, tecnológico, de los profesionales y con procesos bien definidos. Es un tema corporativo y no una prueba, como bien explicó Jorge Juan Fernández y detalló después Júlia Cutillas, cuyo rol, por cierto, es de Community Manager. En el debate salió el tema del retorno de la inversión y ese es un tema inmaduro, ya que es difícil de encontrar métricas adecuadas, pero no dudan de su continuidad ya que forma parte de una estrategia corporativa, en la que siempre hay elementos que forman parte de los costes generales y que se consideran necesarios para prestar el nivel de servicio que se desea ofrecer. Cecilia Pérez desde su posición como Jefe de Implantación de HCE en el Hospital de Móstoles hizo énfasis en la importancia de la gestión efectiva del cambio cuando se implanta un sistema de historia clínica electrónica que pasa por una inicial negación de los usarios al cambio, que luego presentan una resistencia al prinicipio para luego empezar a explorar posibilidades y llegar a un compromiso con el cambio. Santiago Borrás, Jefe de Sistemas del Hospital del Henares, partió de un hospital digital, pero eso no es más que el comienzo. Tras tres años la frustración de los profesionales es no perderse entre demasiada información. La etapa necesaria tras la digitalición es la generación y compartición del cononocimiento. Cristina Ibarrola, Directora de Atención Primaria del SNS-O comentó la experiencia de las interconsultas primaria-especializada que reducen la carga asistencial en primaria al aumentar la resolución. Hay una reserva de tiempos específicos en las agendas de los profesionales de ambos lados para garantizar una respuesta en un máximo de 48 horas. Eso ha llevado a una flexibiliazación de la agenda de los médicos de primaria que tienen un 25% más de tiempo para las consultas presenciales. Parece que aquí la opción tomada es dar más tiempo por paciente en vez de más pacientes, supongo que en parte porque la presión asistencial en Navarra tengo entendido que no es tan fuerte como en otras zonas. Alejandra Cubero comentó la experiencia de identificación de pacientes y de inteoperabilidad en Hospitales de Madrid. Ana Rosa Pulido presentó los logros del SES y su proyecto actual de Imagen Médica No Radiológica. Richard Bernat explicó la experiencia de HCE de Salud de la Mujer Dexeus, indicando que si bien no hay métricas del retorno de la inversión, sí hay una percepción del valor por las diferentes direcciones. Arturo Quesada glosó la experiencia de Jimena en el Hospital de Ávila, Joan Chafer desgranó el arduo proceso de introducción de sucesivas soluciones digitales en el Hospital Clínico San Carlos de Madrid comenzando por “Hogar Digital”, todo ello con financiación externa o recursos propios y cerró el turno de intervenciones no comerciales Pedro A.   • Ajax Autocomplete Extender

    El objetivo de este post es preparar un ejemplo sobre un tema que es planteado muy frecuentemente en los Foros de MSDN, como realizar un Autocomplete contra una base de datos. Qué requerimos? Antes de poder realizar un Autocomplete debemos tener en cuenta los elementos principales que requerimos para poder hacerlo funcionar, descritos de la siguiente manera: 1. Textbox: Nuestro grandioso amigo Textbox, que será donde el usuario ingresará los datos a buscar. 2. Un Webservice: que contendrá el método que se conectara a la base de datos y devolverá una lista con la información encontrada. 3. Ajax Autocomplete Extender: este es por decirlo así, el elemento más importante. Nos servirá como medio de enlace entre el webservice que expone el método y el textbox recuperando y mostrando los datos en forma de lista desplegable. La implementación Si bien parecierá complicado, crear un autocomplete extender es bastante sencillo. Empezaremos creando un nuevo sitio asp.net, en este sitio agregaremos un textbox y dos controles muy importantes de Ajax el ToolkitScriptManager para controlar el rende rizado de los script de ajax y el AutocompleteExtender que, como mencione anteriormente, será el medio de enlace. Antes de mostrar como quedará el código de lo anterior, explicaré algunas propiedades del AutocompleteExtender para que se entienda de mejor manera: 1. El ServicePath: contiene la ruta relativa al webservice que utilizaremos. 2. MinimumPrefixLength: se refiere al número de caracteres que deben ser digitados antes de iniciar la búsqueda. 3. ServiceMethod: el nombre del metodo de nuestro webservice que se encargará de devolver los datos. 4. EnableCaching: para mantener en cache los datos consultados, obteniendo mayor velocidad. 5. TargetControlID: una de las propiedades más importantes, acá se coloca el nombre del textbox al cual se unirá el Autocomplete 6. CompletionInterval: tiempo que debe transcurrir antes de iniciar con el trabajo de los datos. Una vez, explicadas las propiedades básicas, veamos como queda implementada la primer parte de nuestro autocomplete: <form id="form1" runat="server"> <div> <asp:ToolkitScriptManager ID="manager" runat="server" /> <asp:TextBox ID="TextBox1" runat="server"></asp:TextBox> <asp:AutoCompleteExtender ID="AutoCompleteExtender1" runat="server" ServicePath="WebService.asmx" MinimumPrefixLength="1" ServiceMethod="PersonasInfo " EnableCaching="true" TargetControlID="TextBox1" UseContextKey="True" CompletionSetCount="10" CompletionInterval="0"> </asp:AutoCompleteExtender> </div> </form>   Ahora que nuestro código html está completo, es hora de trabajar directamente con nuestro webservice, este deberá contener un método que devuelva una lista o arreglo de datos, los cuales por supuesto, serán traídos desde la base de datos. Una consulta a la base de datos utilizando un datareader y devolviendo los datos en una lista como arreglo. Lo más importante serían las 2 primeras líneas [WebMethod()] y el [ScriptMethod()] las cuales habilitan nuestro método para poder ser accedido y utilizado.   • Not able to use 7-Zip to compress stdin and output with stdout?

    I get the error "Not implemented". I want to compress a file using 7-Zip via stdin then take the data via stdout and do more conversions with my application.   • Reading Data from DDFS ValueError: No JSON object could be decoded

    I'm running dozens of map reduce jobs for a number of different purposes using disco. My data has grown enormous and I thought I would try using DDFS for a change rather than standard txt files. I've followed the DISCO map/reduce example Counting Words as a map/reduce job, without to much difficulty and with the help of others, Reading JSON specific data into DISCO I've gotten past one of my latest problems. I'm trying to read data in/out of ddfs to better chunk and distribute it but am having a bit of trouble.     Hi all, I'm writing some coordinate transformations (more specifically the Joukoswky Transform, Wikipedia Joukowsky Transform), and I'm interested in performance, but of course precision. I'm trying to do the coordinate transformations in two ways: 1) Calculating the real and complex parts in separate, using double precision, as below: double r2 = chi.x*chi.x + chi.y*chi.y; //double sq = pow(r2,-0.5*n) + pow(r2,0.5*n); //slow!!! double sq = sqrt(r2); //way faster! double co = cos(atan2(chi.y,chi.x)); double si = sin(atan2(chi.y,chi.x)); Z.x = 0.5*(co*sq + co/sq); Z.y = 0.5*si*sq; where chi and Z are simple structures with double x and y as members. 2) Using complex : Z = 0.5 * (chi + (1.0 / chi)); Where Z and chi are complex . There interesting part is that indeed the case 1) is faster (about 20%), but the precision is bad, giving error in the third decimal number after the comma after the inverse transform, while the complex gives back the exact number. So, the problem is on the cos(atan2), sin(atan2)? But if it is, how the complex handles that? Thanks!

    I need to extract the name of the direct sub directory from a full path string. For example, say we have: $str = "dir1/dir2/dir3/dir4/filename.ext"; $dir = "dir1/dir2"; Then the name of the sub-directory in the $str path relative to $dir would be "dir3". Note that $dir never has '/' at the ends. So the function should be: $subdir = getsubdir($str,$dir); echo $subdir; // Outputs "dir3" If $dir="dir1" then the output would be "dir2". If $dir="dir1/dir2/dir3/dir4" then the output would be "" (empty). If $dir="" then the output would be "dir1". Etc.. Currently this is what I have, and it works (as far as I've tested it). I'm just wondering if there's a simpler way since I find I'm using a lot of string functions. Maybe there's some magic regexp to do this in one line?     There is a strange thing on my server which has a mysql running on it. The QPS is more than 4000 but TPS is less than 20. The server load is more than 80 and cpu usr is more than 86% but iowait is less than 8%. The disk iops is more than 16000 and util of disk is more than 99%. When the QPS decreases, the load decreases, the cpu iowait increases. The sites that we're hosting are taking a lot time to respond and resolve. top doesn't show anything out of the ordinary and iftop doesn't show a lot of traffic.   • Apache and MySQL taking all the memory? Maximum connections?

    I've a had one of our servers going down (network wise) but keeping its uptime (so looks the server is not losing its power) recently. I've asked my hosting company to investigate and I've been told, after investigation, that Apache and MySQL were at all time using 80% of the memory and peaking at 95% and that I might be needing to add some more RAM to the server. One of their justifications to adding more RAM was that I was using the default max connections setting (125 for MySQL and 150 for Apache) and that for handling those 150 simultaneous connections, I would need at least 3Gb of memory instead of the 1Gb I have at the moment. Now, I understand that tweaking the max connections might be better than me leaving the default setting although I didn't feel it was a concern at the moment, having had servers with the same configuration handle more traffic than the current 1 or 2 visitors before the lunch, telling myself I'd tweak it depending on the visits pattern later. I've also always known Apache was more memory hungry under default settings than its competitor such as nginx and lighttpd. Nonetheless, looking at the stats of my machine, I'm trying to see how my hosting company got those numbers.     I have a Server (2x Hexa-Core Xeon E5649 2.53GHz w/HT with 32GB RAM and 20000 GB Bandwidth) running Ubuntu Server 12.04 LTS. The server runs LAMP and serves one website only, the estimated number of users is to be ~ 15,000 at the same time. At the moment i have around 2000 users online each of them runs 50 MySQL queries (small values mostly select and insert) from the beginning until the end of the session. I'm at a loss to figure out what's going on. How can I find out what's causing this? Update: The system is a fully 64-bit system, so there should be no question of memory limitations due to 32-bit issues. Update2: As I mentioned in the original question, I've already tried tuning swappiness to all sorts of values, including 0. The result is always the same, with approximately 1.6 GB of memory remaining unused. Update3: Added top output to the above info.

The signal used by the timer has been set to SIGUSR1 rather than SIGALRM, since SIGALRM may be used internally by sleep, but it still doesn't seem to work. I have run the program using the command line timer-overruns -d 1 -n 10000000 (1 cs interval) so, in theory, we should expect 100 overruns between calls to sigwaitinfo. However, timer_getoverrun returns 0. I have also tried a version using a time-consuming for loop to introduce the delay. In this case, overruns are recorded. Does anyone know why this happens?
