• Execute remote shell commands on windows XP embedded

  The following situation: We have Windows XP Embedded clients that have all admin shares disabled and only have read-only shares (for security reasons). What we want to do is run remote shell (dos) commands on these machines. At first we looked at PsExec & BeyondExec applications (and all sorts of variants), but all of them rely on having at least an admin$ share, which are disabled on our systems. Telnet is not secure enough, as is RSHD servers. So we looked at the next obvious solution: and SSH server. We also prefer an open-source or freeware solution that is still maintained. I looked at freeSSH server for Windows, but that didn't run stable, I tried installing copSSH, WinSSH & openSSH for Windows, but none of these applications seem to work on Windows XP Embedded. The services can either not be installed or cannot be started. I don't know why. Some kind of dependency that is missing. So are there any other solutions out there? I don't care about having to an agent installation locally of some kind on each system, as long as the size of the software is small enough. Can someone suggest some alternatives to what I've already mentioned? Thank you very much.

• Help, my CentOS servers keep going down , No route to host after a random uptime

  Hello , I have a couple of Centos linux servers, that have a very simple task, they run nginx + fastcgi for php , and some NFS mounts between them, readonly They have some RPC commands to start some downloading processes with wget, nothing fancy , from a main server, but their behavior is very unstable, they simply go down, we tried to monitor ram , processor usage, even network connections, they don't load up so much, max network connections up to... 250 max, 15% processor usage and memory , well, doesn't even fill up, 2.5GB from 8GB max , I have no ideea why can a linux server go down like that, they aren't even public servers, no domain names installed no public serving, for sites. The only thing that I've discovered was that if i didn't restart the network service every couple of hours or so... the servers were becoming very slow, starting apps very slow, but not repoting a high usage of resources...Maybe Centos doesn't free the timeout connections, or something like that...It's based on Red Hat right? I'm not a linux expert , but I'm sure that there are a few guys out there that can easily have an answer to this , or even have some leads to what i can do ... I haven't installed snort, or other things to view if we have some DOS attacks, still the scheduled script that restarts the network each hour should put the system back online, and it doesn't.... • Windows 7 Install: No drives were found

  I was building a computer for my wife with an older SATA hard drive that I had lying around, and when attempting to do a new install of Windows 7 on it, the installer says: "No drives were found. Click Load Driver to provide a mass storage driver for installation." I ran the diskpart command: list volume, and it showed up as "Raw". So, I formatted it to NTFS and then it showed up as a healthy drive in diskpart. I also ran check disk on it with no errors. Windows 7 installer STILL can't find the drive. As far as BIOS settings, I have tried "Native IDE", AHCI, and Both AHCI/IDE mode (SATA slots 0-2 AHCI, 3-4 IDE). I tried all combinations... still "no drives were found". At this point, I'm just scratching my head. Using the installation dos window, I can see and talk to the drive just fine, but the installer just doesn't see it at all. I've even written folders and files to the drive, and it still "can't be seen". Any help would be great. Items of interest: Motherboard model: Gigabyte GA-A75M-UD2H - BIOS Version F5 (latest) Hard drive model: 80GB Seagate Barracuda 7200.7 ST380817AS (no other drives) Installing Windows 7 using a FAT32 formatted USB Drive, which I've used for other installs

• Is there any proper documentation for mod-evasive?

  mod_evasive20 is one of the loaded modules on my httpd server. I read good things about how it can stop a DOS attack and wanted to try it out on my localhost. A search for mod_evasive turns up a blog post by the author which briefly describes what it does. Other than that, I can't seem to find a reference or a documentation on the apache modules site. I was wondering whether it is a module recognised by Apache since there is no mention of it on its website. I have a mod_evasive.conf file sitting in the /etc/http/conf.d folder that contains the following lines: LoadModule evasive20_module modules/mod_evasive20.so <IfModule mod_evasive20.c> DOSHashTableSize 3097 DOSPageCount 2 DOSSiteCount 50 DOSPageInterval 1 DOSSiteInterval 1 DOSBlockingPeriod 10 </IfModule> My understanding from the setting is that if I were to click refresh or send a form more than two times in a one second interval, apache will issue a 403 error and bar me from the site for 10 seconds. • Windows drive letters A: and B:

  This is a question that just popped into my mind and I can't help but wonder why it's still common for a Windows installation to be installed on C: with all other drive letters going up from D: to Z:. In the early MS-DOS times, all we had were floppy disks and they were at A:. When the 3.5 inch floppy started to replace the 5.25 floppy, many people had an A: and B: drive. Then the hard disk became popular and the hard disk was at C: because A: and B: were taken. Then the 5.25 floppy disappeared and most computers had a gap between A: and C:. Nowadays, the 3.5 floppy is just too outdated so A: disappeared too. All disks now start at C:. Yeah, I know I can assign my own drive letters and I've done so with my data disks. My installation disk will just continue to be stuck at C: and I don't really mind. I have no problems with drive letters. But why do the new Windows versions just continue to install themselves by default on C: instead of assigning the letter A: to the boot hard disk?

  I have had problems since my dog pee'd on my computer. Problem: loads windows fine, at random intervals from 5 minutes to 30 minutes it restarts itself. There is nothing in the event log such as errors, no BSOD, just cold restart. after restarting - sometimes- it POST's and restarts itself at the end of POST. It will do this many times and then finally load windows. The cycle then begins again, it will restart eventually. What I have done: I thought it was HDD at first, since this is the only part of the computer which actually got wet with any fluid ( the case is off the PC and the dog pee'd down the front where the HDD is located). Seatool, the seagate HDD tool, found errors when I ran it inside windows, so I ran it in DOS mode from boo-table USB and ran it. It found the same number of errors and fixed them all. I ran the scan again and it says "Good". I loaded windows and ran the scan and it also said "Good there. So the HDD appears to be fine but the problem persists, random restarts. What else could this be? I have taken the computer apart and cleaned everything and also taken the PSU apart and cleaned it thoroughly. The problem still persists, what should my next steps be?

  Hi, How do I make Time Machine to make a new complete backup without losing older versions of backed up files? Verbose: I am using the Time Machine backup on my OS X (Snow Leopard) to backup the whole computer to an external drive. I especially like the "history", i.e. the feature that allows you to restore the older version of a file. Problem: I had some data corruption on my external backup drive, I repaired it with the System Tool for doing that, it found some faults. I had the disk tool repair the external drive. After that, the external drive was OK and I could use Time Machine again. I let Time Machine do one more backup. Now I made a verification according to http://superuser.com/questions/47628/verifying-time-machine-backups, namely along sudo diff -qr . $HOME/Desktop 2>&1 | tee $HOME/timemachine-diff.log However: After doing the command above, several differences and missing files were reported, approx. 200 files in sum. Whereas some of the missing files were cache or excluded directories, the differences do bother me, especially as some important documents from me are listed as differing. How can I make sure that the data on the external drive is synced correctly? Is it possible to have Time Machine to do a complete new backup without losing the history? Or to have Time Machine compare all files for differences and re-write all files that are different? Or can I set some flags on the files that do not match to have them copied again? (like the archive-flag in Windows/Dos). I'd rather not touch the files because I would like to keep the date of last change/date of creation) Thank you for your thoughts!

• Installed Windows 7 Ultimate on D Drive and previous Windows 7 Enterprise on C Drive has stopped starting up

  Please please help! I have installed Windows 7 Ultimate on same hard drive on D Drive on my laptop and the previous Windows 7 Enterprise which was installed on C Drive is not booting up now. When I turn on my laptop, I see two Windows 7 on the screen, when I select newer one, it starts, but when I select older one which is Enterprise edition, system won't start and I get the DOS black screen with this error message: Windows Boot Manager Windows failed to start. A recent hardware or software change might be the cause. To fix the problem: Insert your Windows installation disc and restart your computer. Choose your language settings, and then click "Next." Click "repair your computer." Info: The boot selection failed because a required device is inaccessible. I notice that when I run the newer OS installed, the previous OS's drive (Which is D: now instead of C:) has become unusable and when I double click it, it asks me to format the drive. The data, that I had on my D Drive (Which is now C Drive for new OS), I had copied it to a network path and it is available. It was containing Windows 7 Users folder which I copied at that time when installing new windows. I have copied that Users folder again to the new OS's C Drive thinking it would run again, but of no use. Please please please...if someone can help...It is extremely required for me. • PCI configuration method error (Linux Kernel)

  (I'm not sure if here is the best place for that question, so I will be pleased if anyone suggests me a more proper forum for that.) I'm trying to install Ubuntu 12.04.4 in a netbook (from an usb), but the kernel stops very early in initialization process. After two days of research, I've found that it boots with the parameter pci=conf2 but not with the default conf1 method. Nevertheless, after kernel boot, it seems that Ubuntu can't find usb devices and I'm not be able to install it. Trying with Debian, its a graphic installer and I found that the mouse isn't working neither.I think pci devices are not working. I tried about 50% of kernel pci boot options in the kernel-parameters file (in conjunction with the implicit default conf1) without luck. Any suggestions? PS: The problem is the same with kernel 2.6 or 3. (In Spanish) No estoy seguro si éste es el mejor lugar para esta pregunta, por lo cual estaré encantado si alguno me sugiere un mejor lugar para ella. Estoy intentando instalar Ubuntu 12.04.4 en una netbook (desde un usb), pero el kernel se detiene muy temprano en la inicialización. • [SOLVED} How do I restore my audio after uninstalling Ventrilo?

  Hi, I've a Dell studio 1555 bought on september with Windows 7 64bit Professional on it. The audio device works proprerly, while listening to audio contents (from disk or internet) When I use Ventrilo, the audio from other people sounds good and I hear their voices clearly When I use any other VOIP programs like Teamspeak 3, MSN or Skype, I hear a disturbed voice, and it's impossible to comprehend something... Anyway everything worked fine until I installed Ventrilo, but removing it didn´t solve my problem. Update: Here's a sample of how I hear others people voices.. Audio Sample After some tests, also the desktop has the same problem. So how do I restore my audio? SOLVED The problem was in router firmware, there was a bug that recognized VoIP traffic as a DOS attack and the router grambled every packet... I've installed the newest firmware and everything is fine :)

• Allied Telesis router: IP filtering for the LOCAL interface

  Given an Allied Telesis router with an AlliedWare OS (2.9.1) I would like to disable access to all management services of the router except for a number of subnets (or alternatively have what is a "management VLAN" with other manufacturers' switch and router models). What I have tried so far: creating a new VLAN and an appropriate IP interface, setting the LOCAL IP into this subnet, creating an IP filter for the IP interface and specifying my exclusion subnets: it simply does not work as intended as I can access the LOCAL IP set from any of the other VLAN interfaces - the traffic is apparently not going through my defined filter set at all creating a new IP filter set and binding it to the LOCAL IP interface: this seems not to affect any kind of traffic at all, the counters for the filter set remain at zero packets setting the Remote Security Officer Level IP address range: this only restricts the ability for a user with the Security Officer privilege level to log in from any but the specified address ranges / subnets. Unfortunately, it does not prevent service availability (and thus DoS capacity) or the ability to log in as a less privileged user (e.g. a "manager") calling technical support: unfortunately no solution so far What I have not tried: creating a filter set for each and every IP interface defined on the router and excluding access to the router's management IP: I would like to reduce the overhead induced by IP filters as the router already is CPU-constrained at times. Setting up filters for every IP interface would mean that each and every traffic packet would have to pass the filters, thus consuming CPU cycles. If by any means possible, I would like to find a different solution.

• How To Completely Move Users/Program Files/Program Files (x86)/ProgramData (Folders) To Another Partition(s) On Windows 8?

  I am attempting to move folders Users Program Files Program Files (x86), ProgramData (at the root of the C drive) to at least 2 other partitions, preferably on a fresh install. I have read that there are methods for doing this post-install, but it seems like it would be a bit more tedious to do things that way. I want to move the 2 Program Files folders to another partition on the same HDD, and Users/ProgramData will go to yet another partition on same HDD. I have done a bit of research on this, read up on some things that involved booting into Audit Mode, using the RoboCopy command to copy folders via booting into my Windows 8 USB drive, creating NTFS junctions/symbolic links, Registry edits, as well as accomplishing this automatically by creating an auto-attend file which Windows Setup processes automatically before the user is ever booted in for the 1st time. I tried this morning and now have a basic installation in which programs like Internet Explorer fail to open, certain files can't be found/opened (even if I click on them directly), an example is Regedit. Also, I can't run the Command/DOS (CMD) prompt as Administrator (or otherwise, as any other user), can't activate the real Administrator account or open any of the Administrative Tools (despite having added them to my Start Screen). So far I have only tried RoboCopy-ing Program Files and Program Files (x86) so far, creating junction points for them, and editing the Registry in the relevant locations. This is what I'm left with now. I also found the following blog article which describes how to do this for Windows 7 So, where should I go from here and where can I find more information? And how can this be done without disabling the Metro apps, which I've read will stop working if you move ProgramData. Once I have everything moved, where do I install programs to? Do I tell them to install to C:\Program Files\Program Files (x86) or to the junctioned/symbolic-linked partition/drive? I plan to test in VMware virtual machines from here on until things are working correctly, while using a baseline default install for daily tasks.

• PC hangs and reboots from time to time

  Hello, I have a very strange problem: Since I have my new PC, I have always had problems with it. From time to time the computer freezes for some seconds and suddendly reboots by itself. I've had this problem since Ubuntu 9.10. The same with 10.04 and 10.10. That's why I don't think it's a software failure because the problem persist too long. It doesn't have anything to do with what I'm doing at this time. Sometimes I listen to music, sometimes I only use Firefox, sometimes I'm running 2 or 3 VMs, sometimes I watch DVD. So it's not isolatable. I could freeze once a day or once a week. I put the PC to the vendor twice(!). The first time they changed my power supply but the problem persisted. The second time they told me that they made some heavy performance tests 50 hours long but they didn't find anything. (How can that be that I have daily freezes with normal usage). The vendor didn't check the hard discs because they used their own disc with Windows. (So they never checked the Linux installation). Yesterday I made some intensive hard disc scans with "SMART" but no errors were found. I ran memtest for 3 times but no errors found. I already had this problem in my old flat, so I doubt that I has something to do with current fluctuation. I already tried another electrical socket and changed to connector strip but the problem persists. At the moment I removed 2 of the RAMs (2x 2GB). In all I have 6GB, 2x2GB and 2x1GB. Could this difference maybe be a problem? Here is a list of my components. I hope that anybody find something I didn't think about yet. • Can't Set Crystal Report Selection Formula Programatically

  Hello all, first, I can't stand Crystal! Okay, that's off my chest... Now, we have an old VB6 app we maintain for a client, which uses the Crystal Automation library to programatically change the record selection formulas in a bunch of Crystal Reports 8.5 reports. There are two reports which are ALMOST identical. I had to change them recently to add another field from another table. When I added the table in to the reports though, while it added it in the visual designer, it did not add it in the FROM clause of the SQL statement. So, I manually edited the SQL statement to add in the additional join. KO, works great. If i run the reports in Crystal preview mode, they work exactly as expected. Now, the users went to test the changes from within the VB app. One of the reports works fine and dandy. The other report however, is failing to set the selection formula as expected. The code sets the selection formulas using the function PESetSelectionFormula. I verified that the string being passed in to the function as the new selection formula is correct via a step-through examination of the variables. The call to PESetSelectionFormula seems to be working okay, and is returning a value of 1, which as near as I can find anywhere indicates success. (The other report, which is working fine from code is also returning 1.) However, the report is failing with an error: Error Code: 534 - Error detected by database DLL. The code, for debugging purposes dumps out the SQL string currently being used by the report. The SQL coming out of the report is: SELECT ... FROM ... WHERE ORDER BY ... As you can see, the WHERE clause is blank, which I would imagine is why the database DLL is upchucking on this statement. I don't understand why the automation library is not setting the WHERE clause even though the call to PESetSelectionFormula is being passed a valid string and is returning success. I thought perhaps it was because I had manually edited the SQL in the report to add the table it wasn't adding, but I did the same thing in the other nearly identical report, and that one is working fine. Anyone have any ideas why PESetSelectionFormula might report success but not actually do anything? P.S. I have already tried doing a Database Verify Database from the menus, and that said the report was all up to date and did not help at all.

• Expression Too Complex In Access 2007

  When I try to run this query in Access through the ODBC interface into a MySQL database I get an "Expression too complex in query expression" error. The essential thing I'm trying to do is translate abbreviated names of languages into their full body English counterparts. I was curious if there was some way to "trick" access into thinking the expression is smaller with sub queries, or if someone else had a better idea of how to solve this problem. I thought about making a temporary table and doing a join on it, but that's not supported in Access SQL. Just as an FYI, the query worked fine until I added the big long IFF chain. I tested the query on a smaller IFF chain for three languages, and that wasn't an issue, so the problem definitely stems from the huge IFF chain (It's 26 deep). Also, I might be able to drop some of the options (like combining the different forms of Chinese or Portuguese) As a test, I was able to get the SQL query to work after paring it down to 14 IFF() statements, but that's a far cry from the 26 languages I'd like to represent. • Moving Function With Arguments To RequireJS

  I'm not only relatively new to JavaScript but also to RequireJS (coming from string C# background). Currently on my web page I have a number of JavaScript functions. Each one takes two arguments. Imagine that they look like this: functionA(x1, y1) { ... } functionB(x2, y2) { ... } functionC(x3, y3) { ... } Currently, these functions exist in a tag on my HTML page and I simply call each as needed. My functions have dependencies on KnockoutJS, jQuery, and some other JS libraries. I currently have Script tags that synchronously load those external .js dependencies. But I want to use RequireJS so that they're loaded asynchronously, as needed. To do this, I plan to move all three functions above into an external .js file (a type of AMD "module") called MyFunctions.js. That file will have a define() call (to RequireJS's define function) that will look something like this: define(["knockout", "jquery", ...], function("ko","jquery", ...) {???} ); My question is how to "wrap" my functionA, functionB, and functionC functions where the ??? is above so that I can use those functions on my page as needed. For example, in the onclick event handler for a button on my HTML page, I would want to call functionA and pass two it two arguments; same for functionB and functionC. I don't fully understand how to expose those functions when they're wrapped in a define that itself is located in an external .js file. I know that define assures that my listed libraries are loaded asynchronously before the callback function is called, but after that's done I don't understand how the web page's script tags would use my functions. Would I need to use require to ensure they're available, such as: require(["myfunctions"],function({not sure what to put here})] I think I understand the basics of RequireJS but I don't understand how to wrap my functions so that they're in external .js files, don't pollute the global namespace, and yet can still be called from the main page so that arguments can be passed to them. I imagine they're are many ways to do this but in reviewing the RequireJS docs and some videos out there, I can't say I understand how... Thank you for any help.

  Como el acceso a la documentación está restringido, sólo voy a comentar por encima algunos temas o planteamientos que me han llamado la atención del VI Foro Hospital Digital 2011, organizado por iiR. Y comienzo destacando la buena moderación de Maribel Grau del Hospital Clínic de Barcelona que estuvo sobria, eficaz y motivadora del debate. Me impresionó el proyecto Hospital Líquido del Hospital San Joan de Deu de Barcelona por el compromiso corporativo con una medicina colaborativa involucrando a los pacientes y a los profesionales, con unas iniciativas de eSalud y Salud2.0 avanzadas y apoyadas en un buen soporte legal, tecnológico, de los profesionales y con procesos bien definidos. Es un tema corporativo y no una prueba, como bien explicó Jorge Juan Fernández y detalló después Júlia Cutillas, cuyo rol, por cierto, es de Community Manager. En el debate salió el tema del retorno de la inversión y ese es un tema inmaduro, ya que es difícil de encontrar métricas adecuadas, pero no dudan de su continuidad ya que forma parte de una estrategia corporativa, en la que siempre hay elementos que forman parte de los costes generales y que se consideran necesarios para prestar el nivel de servicio que se desea ofrecer. Cecilia Pérez desde su posición como Jefe de Implantación de HCE en el Hospital de Móstoles hizo énfasis en la importancia de la gestión efectiva del cambio cuando se implanta un sistema de historia clínica electrónica que pasa por una inicial negación de los usarios al cambio, que luego presentan una resistencia al prinicipio para luego empezar a explorar posibilidades y llegar a un compromiso con el cambio. Tras tres años la frustración de los profesionales es no perderse entre demasiada información. La etapa necesaria tras la digitalición es la generación y compartición del cononocimiento. Cristina Ibarrola, Directora de Atención Primaria del SNS-O comentó la experiencia de las interconsultas primaria-especializada que reducen la carga asistencial en primaria al aumentar la resolución. Hay una reserva de tiempos específicos en las agendas de los profesionales de ambos lados para garantizar una respuesta en un máximo de 48 horas. Eso ha llevado a una flexibiliazación de la agenda de los médicos de primaria que tienen un 25% más de tiempo para las consultas presenciales. Parece que aquí la opción tomada es dar más tiempo por paciente en vez de más pacientes, supongo que en parte porque la presión asistencial en Navarra tengo entendido que no es tan fuerte como en otras zonas. Alejandra Cubero comentó la experiencia de identificación de pacientes y de inteoperabilidad en Hospitales de Madrid. Ana Rosa Pulido presentó los logros del SES y su proyecto actual de Imagen Médica No Radiológica. Richard Bernat explicó la experiencia de HCE de Salud de la Mujer Dexeus, indicando que si bien no hay métricas del retorno de la inversión, sí hay una percepción del valor por las diferentes direcciones. Arturo Quesada glosó la experiencia de Jimena en el Hospital de Ávila, Joan Chafer desgranó el arduo proceso de introducción de sucesivas soluciones digitales en el Hospital Clínico San Carlos de Madrid comenzando por “Hogar Digital”, todo ello con financiación externa o recursos propios y cerró el turno de intervenciones no comerciales Pedro A. • Do We Indeed Have a Future? George Takei on Star Wars.

  George Takei (rhymes with Okay), probably best known for playing Hikaru Sulu on the original Star Trek, has always had deep concerns for the present and the future. Whether on Earth or among the stars, he has the welfare of humanity very much at heart. I was digging through my old copies of Famous Monsters of Filmland, a great publication on monster and films that I grew up with, and came across this. This was his reaction to STAR WARS from issue 139 of Famous Monsters of Filmland and was written June 6, 1977. It is reprinted here without permission but I hope since the message is still valid to this day and has never been reprinted anywhere, nobody will mind me sharing it. STAR WARS is the most pre-posterously diverting galactic escape and at the same time the most hideously credible portent of the future yet.While I thrilled to the exploits that reminded me of the heroics of Errol Flynn as Robin Hood, Burt Lancaster as the Crimson Pirate and Buster Crabbe as Flash Gordon, I was at the same time aghast at the phantasmagoric violence technology can place at our disposal. STAR WARS raised in my mind the question - do we indeed have a future?It seems to me what George Lucas has done is to masterfully guide us on a journey through space and time and bring us back face to face with today's reality. STAR WARS is more than science fiction, I think it is science fictitious reality.Just yesterday, June 7, 1977, I read that the United States will embark on the production of a neutron bomb - a bomb that will kill people on a gigantic scale but will not destroy buildings. A few days before that, I read that the Pentagon is fearful that the Soviets may have developed a warhead that could neutralize ours that have a capacity for that irrational concept overkill to the nth power. Already, it seems we have the technology to realize the awesome special effects simulations that we saw in the film.The political scene of STAR WARS is that of government by force and power, of revolutions based on some unfathomable grievance, survival through a combination of cunning and luck and success by the harnessing of technology -  a picture not very much at variance from the political headlines that we read today.And most of all, look at the people; both the heroes in the film and the reaction of the audience. First, the heroes; Luke Skywalker is a pretty but easily led youth. Without any real philosophy to guide him, he easily falls under the influence of a mystical old man believed previously to be an eccentric hermit. Recognize a 1960's hippie or a 1970's moonie? Han Solo has a philosophy coupled with courage and skill. His philosophy is money. His proficiency comes for a price - the highest. Solo is a thoroughly avaricious mercenary. And the Princess, a decisive, strong, self-confident and chilly woman. The audience cheered when she wielded a gun. In all three, I missed qualities that could be called humane - love, kindness, yes, I missed sensuality. I also missed a sense of ideals and faith. In this regard the machines seemed more human. They demonstrated real affection for each other and an occasional poutiness. They exhibited a sense of fidelity and constancy. The machines were humanized and the humans conversely seemed mechanical.As a member of the audience, I was swept up by the sheer romantic escapsim of it all. The deering-dos, the rope swing escape across the pit, the ray gun battles and especially the swash buckle with the ray swords. Great fun!But I just hope that we weren't too intoxicated by the escapism to be able to focus on the recognizable. I hope the beauty of the effects didn't narcotize our sensitivity to violence. I hope the people see through the fantastically well done futuristic mirrors to the disquieting reflection of our own society. I hope they enjoy STAR WARS without being "purely entertained".

• La búsqueda de la eficiencia como Santo Grial de las TIC sanitarias

  Las XVIII Jornadas de Informática Sanitaria en Andalucía se han cerrado el pasado viernes con 11.500 horas de inteligencia colectiva. Aunque el cálculo supongo que resulta de multiplicar las horas de sesiones y talleres por el número de inscritos, lo que no sería del todo real ya que la asistencia media calculo que andaría por las noventa personas, supongo que refleja el global si incluimos el montante de interacciones informales que el formato y lugar de celebración favorecen. Mi resumen subjetivo es que todos somos conscientes de que debemos conseguir más eficiencia en y gracias a las TIC y que para ello hemos señalado algunas pautas, que los asistentes, en sus diferentes roles debiéramos aplicar y ayudar a difundir. En esa línea creo que destaca la necesidad de tener muy claro de dónde se parte y qué se quiere conseguir, para lo que es imprescindible medir y que las medidas ayuden a retroalimentar al sistema en orden de conseguir sus objetivos. Y en este sentido, a nivel anecdótico, quisiera dejar una paradoja que se presentó sobre la eficiencia: partiendo de que el coste/día de hospitalización es mayor al principio que los últimos días de la estancia, si se consigue ser más eficiente y reducir la estancia media, se liberarán últimos días de estancia que se utilizarán para nuevos ingresos, lo que hará que el número de primeros días de estancia aumente el coste económico total. En este caso mejoraríamos el servicio a los ciudadanos pero aumentaríamos el coste, salvo que se tomasen acciones para redimensionar la oferta hospitalaria bajando el coste y sin mejorer la calidad. También fue tema destacado la posibilidad/necesidad de aprovechar las capacidades de las TIC para realizar cambios estructurales y hacer que la medicina pase de ser reactiva a proactiva mediante alarmas que facilitasen que se actuase antes de ocurra el problema grave. Otro tema que se trató fue la necesidad real de corresponsabilizar de verdad al ciudadano, gracias a las enormes posibilidades a bajo coste que ofrecen las TIC, asumiendo un proceso hacia la salud colaborativa que tiene muchos retos por delante pero también muchas más oportunidades. Y la carpeta del ciudadano, emergente en varios proyectos e ideas, es un paso en ese aspecto. Un tema que levantó pasiones fue cuando la Directora Gerente del Sergas se quejó de que los proyectos TIC eran lentísimos. Desgraciadamente su agenda no le permitió quedarse al debate que fue bastante intenso en el que salieron temas como el larguísimo proceso administrativo, las especificaciones cambiantes, los diseños a medida, etc como factores más allá de la eficiencia especifica de los profesionales TIC involucrados en los proyectos. Y por último quiero citar un tema muy interesante en línea con lo hablado en las jornadas sobre la necesidad de medir: el Índice SEIS. La idea es definir una serie de criterios agrupados en grandes líneas y con un desglose fino que monitorice la aportación de las TIC en la mejora de la salud y la sanidad. Nos presentaron unas versiones previas con debate aún abierto entre dos grandes enfoques, partiendo desde los grandes objetivos hasta los procesos o partiendo desde los procesos hasta los objetivos. La discusión no es sólo académica, ya que influye en los parámetros a establecer. La buena noticia es que está bastante avanzado el trabajo y que pronto los servicios de salud podrán tener una herramienta de comparación basada en la realidad nacional. Para los interesados, varios asistentes hemos ido tuiteando las jornadas, por lo que el que quiera conocer un poco más detalles puede ir a Twitter y buscar la etiqueta #jisa18 y empezando del más antiguo al más moderno se puede hacer un seguimiento con puntos de vista subjetivos sobre lo allí ocurrido. No puedo dejar de hacer un par de autocríticas, ya que soy miembro de la SEIS. La primera es sobre el portal de la SEIS que no ha tenido la interactividad que unas jornadas como estas necesitaban. Pronto empezará a tener documentos y análisis de lo allí ocurrido y luego vendrán las crónicas y análisis más cocinados en la revista I+S. Pero en la segunda década del siglo XXI se necesita bastante más. La otra es sobre la no deseada poca presencia de usuarios de las TIC sanitarias en los roles de profesionales sanitarios y ciudadanos usuarios de los sistemas de información sanitarios. Tenemos que ser proactivos para que acudan en número significativo, ya que si no estamos en riesgo de ser unos TIC-sanitarios absolutistas: todo para los usuarios pero sin los usuarios. Tweet
