Best way to distribute form that can be printed or saved?

I need to develop a simple form (intended only for printing) to be filled in by arbitrary end users (i.e. no specialized software). Ideally, I'd like the end-user to be able to save their inputs to the form and update it periodically. It seems that (at least without LiveCycle Enterprise Suite) Adobe Reader won't save data input in a PDF form. Aside from just distributing the form as a Word document, does anyone have any suggestions? Background: I do some work for a volunteer ambulance corps. They have a lot of elderly patients who don't know (or can't remember) their medical history. They want to develop a common form with personal information (name, address, DOB, medications list, etc.) for elderly residents to hang on their refrigerators (apparently a common solution to this problem). As some of them (or their children/grandchildren) are computer literate, it would make most sense to provide a download-able blank form that can be filled in, saved, updated, and re-printed as needed. Due to worries about privacy, HIPAA, etc. anything with server-side generation is out, it needs to be 100% client-side, and in a format that the majority of non-technical computer users can access without additional software. Thanks for any tips... at this point, I'm leaning towards just using a .doc form.

Sorry that I could not compile jsfiddle because it's jsf application hosted locally and code is dependent on data from jsf application. Although I have arrange part of it and part if it as snippet here. Now Everything's correct in Firefox. Suddenly when I open it in chrome something happened. The text on raphael paper suddenly gets scattered in the paper. It's not where it's meant to be. This happens when I convert svg to image and again generate svg. Everything works fine in Firefox. Questions on Juval Lowy's IDesign C# Coding Standard

We are trying to use the IDesign C# Coding standard. Unfortunately, I found no comprehensive document to explain all the rules that it gives, and also his book does not always help. Here are the open questions that remain for me (from chapter 2, Coding Practices): No. 26: Avoid providing explicit values for enums unless they are integer powers of 2 No. 34: Always explicitly initialize an array of reference types using a for loop No. 50: Avoid events as interface members No. 52: Expose interfaces on class hierarchies No. 73: Do not define method-specific constraints in interfaces No. 74: Do not define constraints in delegates Here's what I think about those: I thought that providing explicit values would be especially useful when adding new enum members at a later point in time. If these members are added between other already existing members, I would provide explicit values to make sure the integer representation of existing members does not change. No idea why I would want to do this. I'd say this totally depends on the logic of my program. I see that there is alternative option of providing "Sink interfaces" (simply providing already all "OnXxxHappened" methods), but what is the reason to prefer one over the other? Unsure what he means here: Could this mean "When implementing an interface explicitly in a non-sealed class, consider providing the implementation in a protected virtual method that can be overridden"? (see Programming .NET Components 2nd Edition, end of chapter “Interfaces and Class Hierarchies”). I suppose this is about providing a "where" clause when using generics, but why is this bad on an interface? Need help with jQuery/AJAX call in Safari/Mac

I've got something that works perfectly in FF and MSIE but it's not working properly in Safari. It's a form with selects that get updated via AJAX/jQuery from a MySQL DB. In Safari, when you select the first item, it correctly loads the options for the next select menu; however, when you choose one of those (which loads new options in a subsequent select menu), the whole form resets and is broken from that point on. Does anyone know of a Safari bug that would cause this? Determining selected state of jQuery Buttons

I've got two radio buttons in a .net page which are being transformed to jQuery buttons a la http://jqueryui.com/demos/button/#radio When the page is loaded I have button 2 as checked. When clicking the buttons I'm firing the postback event. Problem is you can click on that button that is selected by default on the initial load i.e. Button 2, the postback is fired but the event handler isn't called in the .net code behind because the radio button is already classed as selected (and in normal circumstances wouldn't allow the postback to fire). To get around this I've added the e.PreventDefault() method BUT this is causing issues when Button 1 is clicked because before the click handler is called the button is set to selected. Hello all, for the last couple of weeks I've finally gotten into Obj-C from regular C and have started my first app. I've watched tutorials and read through a book along with a lot of webpages, but I know I've only just begun. Anyway, for most of the night and this morning, I've been trying to get this code to work, and now that it will compile, I have a few warnings. I've searched and found similar problems with solutions, but still no dice. What I'm trying to do is put an array made from a txt document into the popup list in a combo box. Reading XML or objects from a Web service

This is my first time working with webservices and I am a bit lost. I successfully called the functions, but I only can get one value from the service. I read that the easiest way is to read xml or create objects and then call their values. Currently I use functions that return the desired value but I need to call them 3 times to get all the data witch is a waste of time and resources. I tried to call the service with the URL and use it as a website or getting the service to work the same way without importing into the program. The thing is that i cant find a way to pass the values into the url, because of that i get only blank pages. What is the fastest way to get my data from the services? I need city name, temperature and a flag if the city is valid. I need to pass the zip code to the service. Thank you. jQuery animation loop not working

Hi, I'm trying to create a looping animation that starts on onmousedown and stops on onmouseout. The effect is a simple scroll that will continue looping until you release the mouse. I've created a function which performs the .animate method and it passes itself as a callback but the code only runs once. jQuery / Loading content into div and changing url's (working but buggy)

This is working, but I'm not being able to set an index.html file on my server root where i can specify the first page to go. It also get very buggy in some situations. Basically it's a common site (menu content) but the idea is to load the content without refreshing the page, defining the div to load the content, and make each page accessible by the url. One of the biggest problems here it's dealing with all url situations that may occur. The ideal would be to have a rel="divToLoadOn" and then pass it on my loadContent() function... so I would like or ideas/solutions for this please. Convert SVG to PDF

How would one go about converting a SVG file to a PDF programatically? (I need to alter the SVG in certain respects before generating the PDF so simply pre-converting it using a tool won't be sufficient.) Ideally using Java but Perl or PHP would be fine too. Obviously I am basically considering Apache FOP and Batik with Java. However no matter how long I search I cannot find a simple introduction on how to do it. Things like SVGConverter have descriptions like "Defines the interface for classes that are able to convert part or all of a GraphicContext", but I don't really know what that means. I have this feeling there must be an API to do this quite simply, provided by FOP or Batik, but I'm just not able to find it at the moment (or perhaps it really doesn't exist.) In terms of the supported SVG features I need, the file has some paths which are filled with some linear gradients. Ideally if I could pass the SVG in as a DOM Document that would be ideal; then I would load my template SVG file, change it as specified by the user, and then generate the PDF.

asp.net dynamic HTML form

Hi, I want to create an html page inside a asp.net page using c# and then request that html page. The flow is, I'll be creating a request that will give me a response with some values. Those values will be stored in hidden fields in the html page I'm creating on the fly and then requesting. I figure it would be something like below but I'm not sure if it would work, I've also received some "Thread Aborting" errors. So, does anyone know the proper way to do this or at least direct me to a nice article or something? ERB doesnt get executed in javascript scripts

Hi guys I have a select input on my page. this select input displays/hides fields in the form. This all works fine. But the problem is that if i submit the form and lack some necessary fields, it doesnt set the select to the right value afterwards. Jquery Ajax Search on pressing return key in Internet Explorer

JavaScript onClick() Display

I have an array consisting of several objects containing Strings. I am successfully able to display the array by using: <td><p onclick="theMiddle(this)">The Middle</td> As you see from the td tag this is part of a table. Issue is that the browser opens up a new page to display my text. Are background threads a bad idea? Why?

So I've been told what I'm doing here is wrong, but I'm not sure why. I have a webpage that imports a CSV file with document numbers to perform an expensive operation on. I've put the expensive operation into a background thread to prevent it from blocking the application. Why is this ajax call being made even though it shouldn't be

i have this validation script im working on but cant see why im having an issue. You can see that i have a check = false / true, check before it runs the ajax call. Spreadsheet_Excel_Writer data output is damaged

I use Spreadsheet_Excel_Writer to generate .xls file and it works fine until I have to deal with a large amount of data. On certain stage it just writes some nonsense chars and quits filling certain columns. However some columns are field up to the end (generally numeric data) I'm not quite sure how the xls document is formed: row by row, or col by col... Also it is obviously not an error in a string, because when i cut out some data, the error appears a little bit further. Parsing CSS by regex

I'm creating a CSS editor and am trying to create a regular expression that can get data from a CSS document. This regex works if I have one property but I can't get it to work for all properties. I'm using preg/perl syntax in PHP. Change HTML image into a Button for partial refresh of page

I am new to webpage design and MVC. I have a aspx page and I have a html image. I want to change this image and make it clickable so that it will refresh only the bottom half of the page when clicked. I setup my image so that the source of the image is updated by the controller. By using the following src="<%=ViewData["BookImg"] %>" So that the whole story is clear. When a person clicks on the image of the book, it displays on the bottom half of the page information about that book pulled from a sql database. I am thinking that I would need to look into getting ajax implemented so that I can do partial page update. But the question is how do I change that HTML image into a button. Populating PDF Fields using FDFACX

I was recently asked to preform some updates to an existing PDF document. The changes required were completed using Adobe Designer (the only tool I have available to me). These changes included alignment, and new text. Note that there were fillable form fields on the forms, and they were left untouched. The saved version of the form was then put into our ASP.NET application, which pre-populates the form fields were applicable (things like name, address etc... things we have in our database). For some reason, the new form does not populate. I've confirmed that the form fields have the correct names, that the actual file (the pdf) that is being pre-populated has the same permissions as others that are working. There are no errors thrown, and no difference in a step through with a working form and a non-working form. This is a legacy project and I have no real experience with the PDF populator they are using ... FDFACX .NET? And can't find a lot of info on it online. Any ideas?
