• What are the Worst Software Project Failures Ever?

  Is there a good list of "worst software project failures ever" in the history of software development? For example in Canada a "gun registry" project spent around two billion dollars. (http://en.wikipedia.org/wiki/Gun_registry). This is of course, insane, even if the final product "sort of worked". I have heard of an FBI Case file system which there have been several attempts to rewrite, all of them so far, failures. There is a book on the subject (Software Runaways). There doesn't seem to be be a software "boondoggle" list or "fiasco" list on Wikipedia that I can see. (Update: Therac-25 would be the 'winner' of this question, except that I was internally thinking more of Software projects that had as their deliverable, mainly software, as opposed to firmware projects like Therac-25, where the hardware and firmware together are capable of killing people. In terms of pure software monetary debacles, which was my intended question, there are several contenders.)

• Starting a VC++ .exe from MSAccess

  For a complex calculating problem we use an VC++ module (.exe). For the users we have build an ms Access application as userinterface. After input dthe calculating module should start and do its job. The result is again imported in Ms Access. However we work wit dBase files (.dbf) as datafiles. The calculating unit works fine by its own, but when access is running it wo'nt do its job. It will start-up when called from the Access module, but then generates errors or at least does not write correct results to a datatable. We cannot find the reason for this beheavior. Can anyone give us a tip?

• Javascript Onclicks not working?

  I have a jQuery application which finds a specific div, and edit's its inner HTML. As it does this, it adds several divs with onclicks designed to call a function in my JS. For some strange reason, clicking on these never works if I have a function defined in my code set to activate. However, it works fine when calling "alert("Testing");". I am quite bewildered at this as I have in the past been able to make code-generated onclicks work just fine. The only thing new here is jQuery.

  I've noticed that google app engine seems to have a fair amount of downtime where they place the datastore into read-only mode. Frequently this downtime is in the middle of the day. Is this something that is happening only during early development, or is this something that I can expect to be always be occurring? I've developing an application that helps small businesses handle their operations. One thing that it does is take appointments, another is route phone calls. I'd like some suggestions on how to handle times when the datastore is in read-only such as: What if our client is on the phone with the customer and is taking down an appointment and the datastore is in read-only? It would not be acceptable to ask the client to come back later to save, especially if its in the middle of the day. What if there is an incoming call and the application can not store the record or properly route the call due to database writes being unavailable? How are these types of issues normally handled?

• manage.py runserver not working

  I am new to django and python in general, so pardon me for any simple mistakes I may be doing. I am trying to setup my first django project on my local windows vista machine. I have created the project successfully with no problems. The issue I am coming across is when my settings.py has values for my database keys, the manage.py runserver command is failing. If I have values in settings before I run the command, as soon as I run it I get errors. If I have already run the command and the server is running, as soon as I edit the settings file with values, the errors show up in my still open command prompt. The inner most exception seems to "Error loading MySQLdb module: No module named MYSQLdb". If I leave the settings.py blank, the command executes with no problems. Any advice would be greatly appreciated! Thanks

• Excel 2007 UDF keeps failing, why?

  I've been trying to browse info about this for a while now. The user defined functions in Excel 2007 just make no sense to me whatsoever. According to all the tutorials, it should be really simple. Just press alt + f11, insert a new module, and enter the VBA code there. At this point typing '=FUNCTIONNAME(parameters)' into any cell should make it work. It did, at first. Then upon closing and reopening the worksheet it stopped working. I checked to see that macros were enabled, but either way, that didn't help. Then I browsed the graphic UI from the function button to locate user defined functions. I found it there by name of 'MyExcelWorksheet.xls!FUNCTIONNAME.FUNCTIONNAME' and clicking that, it started working again. Now, a day later, I get back to working on this and it's broken, again. I just get #NAME? error or some "Function Arguments - Function doesn't use any arguments" -popup (translated from Finnish Excel, not sure what that msg is in English) when I select the function from the GUI. So what does it take to just create a function, and actually make it work and KEEP working? For the life of me I can't figure out why this has to be so difficult. :) I'm starting to think my company has some weird settings enabled or whatever, because judging by the lack of information I've found on the subject, this isn't supposed to happen.

• Tomcat startup fails with not a valid identifier

  I have tomcat 6.0.18 running on one server without a problem. With the exact same settings it fails to launch on my colleague's machine. He's even running from the same folder as me (I've stopped my copy while he tries to make it work) All we get when we fire off tomcat using bin/startup.sh is this: CATALINA_OPTS=-server -Xms768m -XX:+UseParallelGC -Xmx768m -XX:MaxPermSize=256m -XX:PermSize=128m -Djava.awt.headless=true: is not an identifier I had that definition in setenv.sh and moved it into startup.sh - same problem. Any suggestions? My brief look on google seem to indicate multiple IP address issues, but my server has two ethernet cards, and two IP addresses. Thanks.

  Are there any fixes for this? This issue effects a machine recently upgraded to sp3 from sp2. CSS images and backgrounds not rendered on yahoo mail or wellingtonpeoplescentre.org.nz. IE8 is a recent upgrade from IE7. FF3.6.3 is a fresh install. firebug proves .css is received by FF3. Owner is kind of freaked by what she is seeing.

We just had a second assertion failure (different "in file", position, etc) while running a large set of inserts. After the first failure, MySQL restarted after a reboot only - after continuously looping on the same error after trying to recover. I decided to do a mysqlcheck with -o for optimize. Since these are all Innodb tables (very large tables, 60+GB) this would do an alter table on all tables. • What failure can kill a long running IRC client? [closed]

  I have an IRC bot that I built in PHP using sockets that attempts to run forever and (if disconnected) reconnects again. I have it listening to several channels. Apparently it's fairly resilient, because it can run for several days before the process ends and CRON has to start it up again. However, based on the fact the process ends I'm assuming there are other conditions I'm not accounting for that are causing problems. I have nothing in my error logs giving me a hint. In addition, sometimes the process will continue running - but I notice it's no longer present in any of the channels on the IRC server which makes me think it violated some part of the protocol. I have logic setup to handle: reply to PING's correctly reconnect on disconnect (and reconnect to channels) respond to private messages (so someone doesn't ban it) prevent memory leaks What other failure could be killing my long-running IRC client?

The easiest way to solve this problem is to create a new one. Navigate to:  Microsoft Visual Studio 2010>Visual Studio Tools>Visual Studio x64 Win64 Command Prompt (2010)  [if you aren't on an x64 box, pick another command prompt option that fits] Once the MS-Dos window displays, type in this statement: sn -k c:\SampleKey.snk Then copy the output *.snk file to project directory, or the *referenced directory. Remove the old reference to the *.snk file from the project. Add the paired key back to the project as an existing item. When you add back the *.snk file to the project, you will see that the *.snk file is no longer missing.   Our work is done!   *referenced directory: Pay attention to the original error message on compile. The *.snk file that is referenced may be in a directory path you aren't expecting--so you will still get the error unless you change the directory path or write the file to the directory the program is expecting to find the *.snk file.

  Introduction There is nothing more frustrating than a problem that "cannot be reproduced". Logs, configuration files have been analysed but there just isn't enough information to establish the root cause. The issue maybe closed, but you are left with the feeling that the problem will raise its ugly head again in the future. Trouble is, to resolve such issues you need to capture diagnostic data at the exact time the incident occurs. Step forward Fusion Middleware Diagnostic Framework!  Diagnostic Framework monitors WebLogic Managed Servers and delivers "Automatic capture of diagnostic data upon first failure". To quote fromOracle Fusion Middleware Administrator's Guide 11g Release 1 (11.1.1)Chapter 13 Diagnosing Problems "When a critical error occurs ... the Diagnostic Framework automatically collects diagnostics, such as thread dumps, DMS metric dumps, and WebLogic Diagnostics Framework (WLDF) server image dumps ... The data is stored in a file-based repository and is accessible with command-line utilities." In other words the data collected upon first failure - especially the thread and image dumps - provides a snapshot of the system as or immediately after the problem occurs. The table below shows the type of WebLogic Server issues which fall into the scope of Diagnostic Framework How to Configure Diagnostic Framework? Depending on your Fusion Middleware product choice you may not need to do anything! This template is applied by default whenever you configure WebLogic Managed Servers for products such as Portal / Forms / Reports / Discoverer Identity Management ( OID , OAM , OIM etc) WebCenter SOA Check your WebLogic Domain directory structure. If you have an "adr" sub directory under DOMAIN_HOME/servers/<servername>/ then JRF template has been applied and Diagnostic Framework will be in play. How to Upload Diagnostic Framework Incident Data to Oracle Support Some Background Information There are three interfaces to the Repository: Enterprise Manager Cloud Control (Support Workbench) WLST (Command Line) ADRCI (Command Line) The Enterprise Manager Cloud Control does provide a nice GUI interface to search, view and package diagnostic framework incidents. However, this software is not to be confused with Fusion Middleware (EM) Control. Cloud Control (formerly known as Grid Control) is part of the Enterprise Manager media package. EM Cloud Control has it's own install and configuration story. Therefore, for the benefit of those yet to install and play with Cloud Control, I am going to describe how to use the command line tools. Ideally, you would only need to one command line interface, but currently I suggest using both - mainly due to the fact that ADRCI SHOW INCIDENTS does not reveal the description behind the Diagnostic Framework error code. Instructions: Note: WLST and ADRCI are case sensitive when it comes to handling parameter values. • SMART says disk failure is imminent due to bad blocks, what do I need to do?

  I have on my hard drive 2 OSes: Ubuntu 12.04 and Windows Vista (I keep it just because of school). Everything was OK on both OSes, but one day on Ubuntu I was getting awkward noises from my notebooks' hard drive and then everything stopped and I couldn't do anything. On Windows everything was OK. Every time I boot Ubuntu I can get 5 minutes normal run time, without problems. After that the hard drive sounds crazy and nothing works. I could run S.M.A.R.T tests from a older Ubuntu CD (10.04) from the GUI (Disk Utility, or something like that and from terminal). From the GUI, I got that the DISK FAILURE IS IMMINENT and I have ~700 bad blocks (or broken blocks, I had that test I while ago) on my HDD. From the terminal (I don't remember if it was fsck or a SMART test command) I got that the HDD will fail in under 24 hours. Since then it passed 2-3 weeks. I've tried "badblocks" but after 10 hours it was still running and I had to stop it. Now I have to use cygwin and other alternatives for my Linux apps on Windows. How can I separate the bad blocks from Ubuntu so it wouldn't use them? Please help.

• Apt-Get Update: failure to fetch; can't connect to any sources

  I realize there are dozens of "apt-get update: failure to fetch" questions (I read through all I could find), but my present circumstance is unique to 12.04 and it affects all sources; not just launchpad. Additionally, I've tried several different servers in Europe and the U.S. as well as the "main server" (wherever that is) and they all yield the same result: I can't connect to any software sources. Additionally, I'm fairly certain the problem stems from the upgrade from 11.10-12.04 I performed this morning, as updates worked immediately before. Updates from the Update Manager worked fine and I could download some things (mutter) from the Software Center without incident, which makes me think I can connect to some subset of the Ubuntu servers (however, several other Ubuntu servers--like extras--and some canonical servers are listed as 'unable to connect'). • 12.04: Apt-Get Update: failure to fetch; can't connect to any sources

  I realize there are dozens of "apt-get update: failure to fetch" questions (I read through all I could find), but my present circumstance is unique to 12.04 and it affects all sources; not just launchpad. Additionally, I've tried several different servers in Europe and the U.S. as well as the "main server" (wherever that is) and they all yield the same result: I can't connect to any software sources. Additionally, I'm fairly certain the problem stems from the upgrade from 11.10-12.04 I performed this morning, as updates worked immediately before. Updates from the Update Manager worked fine and I could download some things (mutter) from the Software Center without incident, which makes me think I can connect to some subset of the Ubuntu servers (however, several other Ubuntu servers--like extras--and some canonical servers are listed as 'unable to connect'). • How to handle server failure in an n-tier architecture?

  Imagine I have an n-tier architecture in an auto-scaled cloud environment with say: a load balancer in a failover pair reverse proxy tier web app tier db tier Each tier needs to connect to the instances in the tier below. What are the standard ways of connecting tiers to make them resilient to failure of nodes in each tier? i.e. how does each tier get the IP addresses of each node in the tier below? For example if all reverse proxies should route traffic to all web app nodes, how could they be set up so that they don't send traffic to dead web app nodes, and so that when new web app nodes are brought online they can send traffic to it? I could run an agent that would update all the configs to all the nodes, but it seems inefficient. I could put an LB pair between each tier, so the tier above only needs to connect to the load balancers, but how do I handle the problem of the LBs dying? This just seems to shunt the problem of tier A needing to know the IPs of all nodes in tier B, to all nodes in tier A needing to know the IPs of all LBs between tiers A and B. For some applications, they can implement retry logic if they contact a node in the tier below that doesn't respond, but is there any way that some middleware could direct traffic to only live nodes in the following tier? If I was hosting on AWS I could use an ELB between tiers, but I want to know how I could achieve the same functionality myself. I've read (briefly) about heartbeat and keepalived - are these relevant here? What are the virtual IPs they talk about and how are they managed? Are there still single points of failure using them?
